Description
COMBINED SYNOPSIS/SOLICITATION TEMPLATE DESCRIPTION: This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation number F6016032090100 is issued as a request for quotes(RFQ) and will result in a Firm-Fixed Price Supply. Award will be made on an all or none basis. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-15. This acquisition is being acquired as a Small Business Set-Aside. The SIC code is 1752, NAICS 235520 and the small business size standard is 500. The contract line items numbers and descriptions are: CLIN 0001 ? Removal and installation of Carpet. Description of Requirement/ Statement of Work (SOW): Remove old carpet and base; dispose off-site, clean floors, minor floor preparation, supply and install new carpet with special adhesive. Supply and Install 4? cove base. Remove and replace all furniture in all areas. The Contractor shall provide all labor, equipment, and transportation to perform all operations necessary to remove and replace all existing carpet in suites occupied by Air Force Legal Services Agency personnel only. Areas to receive carpet are as follows: (a) Suite 336 ? Carpet ? 1655 sq ft (Base 350lin ft) (b) Suite 343 ? Carpet ? 1305 sq ft (Base 400lin ft) (c) Common Hall ? Carpet-900 sq ft (200 lin ft) (d) Suite 335 ? Carpet ? 250 sq ft (Base 100 lin ft) (e) Suite 301 ? Carpet ? 675 sq ft (Base 200 lin ft) (f) Suite 303 ? Carpet ? 540 sq ft (Base 200 lin ft). Carpet Type: Monterey Mills (California). Quality: 2005 VIP Traditional. Style: 6040 Marbella II. Color: 797 Avila. Special Backing- Please provide wear warranty terms. 12? Broadloom. Delivery and Acceptance shall be 30 September 03. after contract award. Award date is expected to be (29 Sept 03). An award will be made to the offeror who submits a responsive quote for all items listed at the lowest cost to the Government. Offerors shall provide back-up information supporting the price offered. This could take the form of the most recent catalog price list, invoices from previous sales of same items, etc. Inspection and acceptance shall be made at destination. The contractor shall extend to the Government full coverage of any standard commercial warranty normally offered in a similar commercial sale. Acceptance of the warranty does not waive the Government?s right under the Inspection clause, nor does it limit the Government?s rights with regard to the other terms and conditions under this contract. In the event of a conflict the terms and conditions of the contract shall take precedence over the warranty. The warranty period shall begin upon final acceptance of the applicable deliverables listed in the schedule. FAR 52.212-1, Instructions to Offerors-Commercial, and FAR 52.212-2 Evaluation - Commercial Items, are incorporated by reference and apply to this acquisition. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representation and Certifications - Commercial Items and DFAR 252.212-7000 Offeror Representations and Certifications-Commercial Items with its offer. The FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items is hereby incorporated by reference. The clauses at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items apply to this acquisition, specifically, the following cited clauses are applicable52.203-6, 52.203-10, 52.219-8, 52.219-9, 52.219-14, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-3, 52.225-9, and 52.225-21). Responses to this solicitation are due by (1200hrs) on (29 Sept 03). The proposal shall be signed by an official authorized to bind the company.
