- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotes (RFQ). Submit written offers (oral offers will not be accepted), on RFQ F7HOS232200600. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-14 and Defense Acquisition Circular 91-13. This procurement is being issued as 100% small business set-aside. The North American Industry Classification System (NAICS) Code is 561421 and the size standard of $6,000,000.00 applies to this procurement. The requirement is for TRICARE answering service (see attached statement of work). The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this acquisition. The evaluation will be based on total cost. The clause at FAR 52.212-3, Offer Representation and Certifications ? Commercial Items applies. Contractors shall submit the representations and certifications with their proposal. The clause at FAR 52.212-4,Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at FAR 52.212-5 (Dev), Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items, applies to this solicitation. Specifically, the following clauses cited are applicable to this solicitation: 52.232-33, Payment by Electronic Funds Transfer--Central Contractor Registration, and 52.252-2, Clauses Incorporated by Reference. Wage determination 94-2023, revision 26 applies to this solicitation. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the DoD Central Contractor Registration (CCR) database, NO EXCEPTIONS. Quotes are required to be received no later than 4:00 PM AM PST, 21 Sep 03. Statement of Work Services, Nonpersonal. Contractor shall provide all management, tool, supplies, equipment and labor necessary to provide 24-hour answering service, seven days per week for the on-call Luke Primary Care Manager (PCM) teams. This contract will consist of one base year (1 Oct 03 ? 30 Sep 04) and three option periods (1 Oct 04 to 30 Sep 05) (1 Oct 05 ? 30 Sep 06) (1 Oct 06-30 Sep 07). Tasks include: a. Provide a live operator to answer incoming phone calls from Luke TriCare Prime beneficiaries. Pre-screening will be accomplished prior to the calls being forwarded to the answering service. b. Provide 24 hour answering operations. c. Provide a faxed report, on a daily basis, to the appropriate PCM which will show caller?s name, phone number, sponsor?s Social Security number, Name of PCM, patients name, (last, first, MI), age, reason for call, Date, Time call received, time PCM responded to answering service; d. Page the appropriate PCM on call within five minutes after the patient has contacted the answering service; provide the on-call PCM the patient?s name, Social Security number, reason patient called, and telephone number to reach patient; e. Maintain a roster of providers? on-call, provider pager numbers, and provider home phone numbers; f. The contractor will provide PCM Point of Contacts (POCs) with a fax number to fax call schedule to the Provider?s POCs g. The contractor will at no time will contractor give out medical advice to a patient. If the patient states they have an emergency and has still gone through all voice prompts to get to the answering services and states they cannot wait to speak to their PCM, contractor will accomplish the following: The patient will be instructed to if they have an emergency to go to the nearest Emergency Room or call 911 is assistance is required. This information will be logged on the call sheet with the patients name and number, if provided h. Contractor will provide on a monthly basis by the 15th of each month, a roster of total calls. Military Treatment Facility (MTF) must notify the contractor on a monthly basis, prior to the 15th of each month, in order to receive this roster; i. On a monthly basis, the MTF Primary Care Manager POC will provide the contractor the PCM on-call schedule for their Luke TriCare Prime Primary Care Managers. The listing should include provider first and last name, rank, or professional status (i.e. Dr., PA, etc). GOVERNMENT FURNISHED PROPERTY/SERVICES/EQUIPMENT: Pagers and cellular phone for the PCMs will be provided by Luke Hospital. Providers will need to indicate on the call schedule their preference of contact method and timeframe in order for answering services to comply provider preferences (i.e. on beep until 9 p.m. each time on call, the use home phone, etc); the contractor will utilize a Qwest provided phone line (direct line and 1-888) provided by the Hospital for the Answering Service; The MTF PCM POCs will provide all newly assigned providers with a card from the ?contractor? which instructs the provider on how to reach the answering service and what code needs to be provided in order for their messages to be pulled from the computer system. If providers have any difficulties with the service, it provides them a number to contact management at the service.
