Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Purchase Request number is FH330032411400 as a Request for Quote using Simplified Acquisition Procedures. The solicitation document and incorporate provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-15. It is the contractor’s responsibility to be familiar with applicable clauses and provisions. This acquisition is for the purchase of Catholic Auxiliary Chaplain for the Maxwell AFB/Gunter Annex Chapel. This request is unfunded and will be awarded pending approval of funding. This acquisition is 100% set-aside for small business. The North American Industrial Classification System Code is 611699 with a small business size standard of $6,000,000.00. The following clauses and provisions are incorporated either by reference or full text and are to remain in full force in any resultant purchase order; FAR 52.212-1, Instructions to Offerors, Commercial Item (Oct 2000) applies to this acquisition. Submit signed and dated proposals on company letterhead. The Government will make an award to the lowest priced, responsible, contractor whose proposal conforms to the solicitation. The contractor must be registered in the Central Contractor Registration database. Offerors must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Jul 2002) and DFARS 252.212-7000, Offeror Representations and Certifications—Commercial Items (Nov 1995) with the proposal. Clauses and provisions may be accessed via the internet, http://farsite.hill.af.mil. The clause at FAR 52.212-4 (Feb 2002), Contract Terms and Conditions-Commercial Items is incorporated into this RFP; 52.219-6, Notice of Total Small Business Set-Aside and 52.219-6 Alternate I, Notice of Small Business Set-Aside (Jul 1996). FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (May 2002) is applicable. Quotes are required to be received no later than 4:00 PM CST, Friday, Sep 26, 2003 STATEMENT OF WORK AUXILIARY CATHOLIC CHAPLAIN MAXWELL/GUNTER AFB, AL GENERAL DUTIES OUTLINE: The Auxiliary Catholic Chaplain will be responsible for providing ministerial services for the immediate needs of the Maxwell AFB Catholic Parish. Services and duties shall include, but are not limited to the following: CONTRACTOR SHALL: 1. Be able to perform the following as requested/required at Maxwell AFB and/or Gunter Annex: (a) Saturday Vigil, Sunday, Holy Day Vigil ,or Holy Day Mass, including the performance of the Sacrament of Reconciliation for one hour before Mass (70 Units) (b) Emergency Catholic Sacramental Ministrations outside of Mass and/or Pastoral Counseling. (5 Units) (c) Weekday Mass to include performance of the Sacrament of Reconciliation for a half (1/2) hour after Mass. (15 Units) 2. May provide a substitute of equal or higher proficiency. All substitutes shall be suitable to the Catholic Chaplain, or the Wing Chaplain in his absence, and approved five (5) days in advance. 3. The above tasks are subject to coordination with appropriate Chapel Staff and monitoring by the Catholic Chaplain who will serve as the Quality Assurance Evaluator. In the absence of the Catholic Chaplain, the Wing Chaplain will assign another staff member to serve as Quality Assurance Evaluator. 4. Contractor shall submit invoices to 42 ABW/HC ATTN: Resource Advisor no later than seven (7) days after the last billiable unit. REQUIREMENTS/QUALIFICATIONS: 1. Shall be an ordained Catholic Priest in good standing with the Roman Catholic Church and provide a letter from his Bishop and/or Superior stating so. 2. Shall be able to speak and write English fluently. 3. Shall have at least 10 years of previous experience working with a Military Catholic Chapel Program. 4. Shall have a record of effective communication and good working relationship with chaplains and/or chapel staff. 5. Shall be able to work closely with the Catholic Chaplain, Chapel Staff, and parishoners to develop good rapport within the Catholic Parish. 6. Shall possess and demonstrate the ability to work cooperatively with chapel staff, choir directors, musicians, choir members, parishoners, and members of other faith groups.
