- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation 552-49-03 is being issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-15. NAICS Code 333312, 500 employees small business size standard applies. Items will be purchased and delivered to the VA Medical Center, 4100 W. Third St., Dayton, Ohio 45428. Prices shall be submitted for the following commercial laundry equipment and related parts/services: 1) 1 ea. Spreading and Feeding device, laundry, commercial Jensen Model Quick Feeder – quick feed clampless spreader/feeder, 2-stations, 120” working width, PLC microprocessor, clampless design, ergonomic design on feeding stations with 3-position height adjustments, automatic speed control, production counter, spreading by vacuum, vacuum for leading side and rear edges, automatic rejection for misfeeds, inlet conveyor drop down for feeding of wax cloth, fast disconnect airlines, electric 208-240/60/3; 1.a.) Warranty: one year parts and labor; 1.b.) 2 ea. Operator’s manuals; 1.c.) 1 ea. Service manual containing parts list, schematics and wiring diagram; 1.d.) Trade in of old equipment- 1 ea. Jensen Model 288 S/N: 10009 - in the form of a credit; 1.e.) Installation-including delivery, uncrating, assembly, hook-up to proper existing utilities, start up testing and training; 2) 2 ea. Washer-Extractor, commercial 600 lb capacity G.A. Braun 600NTDP-3; 2.a.) Warranty: one year parts and labor; 2.b.) 2 ea. Operator’s manuals; 2.c.) 1 ea. Service manual containing parts list, schematics and wiring diagram; 2.d.) Trade in of old equipment – 2 ea. 600lb Washer/Extractor G.A. Braun NTD600 S/N’s: NTD60077309 & NTD60077311- in the form of a credit; 2.e.) Installation-including delivery, uncrating, assembly, hook-up to proper existing utilities, start up testing and training; 3) 1 ea. Dryer-Tumbler, commercial 440 lb capacity G.A. Braun 440 Dryer; 3.a.) Warranty: one year parts and labor; 3.b.) 2 ea. Operator’s manuals; 3.c.) 1 ea. Service manual containing parts list, schematics and wiring diagram; 4.d.) Trade in of old equipment – 1 ea. 440lb Dryer Tumbler G.A. Braun 440PSH SN: PSH44099001- in the form of a credit; 3.e.) Installation-including delivery, uncrating, assembly, hook-up to proper existing utilities, start up testing and training; 4) 1 ea. Laundry Folding Machine Jensen “Silverline Plus” Model 4213 – Four lane folder with single lane 3 cross folds, PLC computer microprocessor, self-cleaning photo cells, last crossfold by knife, automatic belt tensioning, jam alarm, fast disconnect airlines, complete interlock safety guards and stop buttons, 120” working width, 208-240/60/3; 4.a.) Warranty: one year parts and labor; 4.b.) 2 ea. Operator’s manuals; 4.c.) 1 ea. Service manual containing parts list, schematics and wiring diagram; 4.d.) Trade in of old equipment – 1 ea. Folder Cross Folder, G.A. Braun Model 4PF3CF S/N: OML1190431282 - in the form of a credit; 4.e.) Installation-including delivery, uncrating, assembly, hook-up to proper existing utilities, start up testing and training; 5) 1 ea. Laundry Folding Machine Jensen “Silverline Plus” Model 4213 – Four lane folder with single lane 3 cross folds, PLC computer microprocessor, self-cleaning photo cells, last crossfold by knife, automatic belt tensioning, jam alarm, fast disconnect airlines, complete interlock safety guards and stop buttons, 120” working width, 208-240/60/3; 5.a.) Warranty: one year parts and labor; 5.b.) 2 ea. Operator’s manuals; 5.c.) 1 ea. Service manual containing parts list, schematics and wiring diagram; 5.d.) Installation-including delivery, uncrating, assembly, hook-up to proper existing utilities, start up testing and training. Offeror’s wishing to view the VAMC laundry prior to submitting their quote may do so by attending a site visit to be conducted on September 12, 2003, at 2:00 PM, in Bldg. 143. The following provisions and clauses apply to this solicitation: 52.212-1 Instructions to Offeror-Commercial Items, 52.212-4 Contract Terms and Conditions-Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Order Commercial Items with (b) (1),(14),(15),(16),(17),(18),(19),(21),(24), and (30) checked. FAR 52.233-2 Service of Protest, VAAR 852.233-70 Protest Content, VAAR 852.233-71 Alternative Protest Procedure and VAAR 852.270-4 Commerical Advertising also apply to this solicitation. Quotes will be evaluated on the following criteria: technical capability of equipment, price and service agreement offered. Award will be made to the offeror whose products meet the requirements listed above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. The Government reserves the right to make one, or multiple awards on the above equipment and related supplies/services. Offers including the pricing information are to be submitted on company letterhead and include the information required in provisions 52.212-1, a completed copy of the provisions 52.212-3 Offeror Representations and Certifications-Commercial Items, literature/brochures on proposed equipment and the company DUNS number. Electronic copies of the referenced provisions and clauses are available on the Internet at www.arnet.gov/far and www.va.gov/oa&mm/vaar/. Quotations and all required information must be received by 2:30 PM local time on September 19, 2003. Quotations shall be submitted to Jodi A. Cokl, Contracting Officer, VA Medical Center, 4100 W. Third Street, Dayton, Ohio 45428. Faxed quotations are acceptable and may be faxed to (937) 262-5974. Solicitation number must be listed on all documents for identification purposes. Only offers prepared in accordance with this notice will be considered.
