Description
This is a combined synopsis/solicitation for commercial items prepare= d in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-15. The North American Industry Classification System Code is 492110, Couriers. The small business size standard is 1,500 employees. This procurement is a total small business set-aside. Offerors are encouraged to submit a per trip price quote and total cost quote based on an estimated number of 250 per year, as well as an aggregate total cost for a base year plus four pre-priced option years. Proposed rates shall include all costs associated with providing the following services. Request For Proposals (RFQ) 515-04-04, C.0. Scope: The successful offeror shall provide all vehicles, gasoline, insurance, personnel, directional materials to necessary to accomplish regularly scheduled courier services for and between the Department of Veterans Affairs Medical Center (VAMC), Battle Creek, Michigan and the Department of Veterans Affairs Community-Based Outpatient Clinic (CBOC), Benton Harbor, Michigan. Services shall be provided Monday through Friday, excluding government holidays, for a base year from October 1, 2003 through September 30, 2004, plus four pre-price option years from October 1, 2004 through September 30, 2008. C.1. 0=20 Schedule/Locations/Items: C.1.1. Work Hours - Pick up at the Department of Veterans Affairs Community-Based Outpatient Clinic, currently located at 960 Agard, Benton Harbor, MI 49022 between 4:30 p.m. and 5:00 p.m. (Note: The clinic will be relocated to 115 West Main Street, Benton Harbor, MI 49022 effective November 1, 2003) and delivery shall be made no later than 7:00 p.m. to the VAMC, 5500 Armstrong Road, Battle Creek, Monday through Friday, excluding National Holidays. C.1.1.1. National Holidays: The ten (10) holiday observed by the Government are: New Years Day, President's Day, Martin Luther King's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, veterans Day, Thanksgiving Day, Christmas Day and any other day specifically declared by the President of the Untied States to be a national holiday. C.1.1.2. Insurance - The Contractor shall have the sufficient insurance on contractor owned vehicles and personnel. It should be noted that blood substances will be one of the items transported by the contractor. C.1.1.3. Inclement Weather Provision - The Contractor shall contact the CBOC Coordinator or designee when the Contractor feels that weather indicates adjustment or cancellation of scheduled pick up/deliveries. C.1.1.4. If the contractor fails to furnish the required services within the time frames established, the Department of Veterans Affairs reserves the right to obtain the service from another source and to charge the contractor with any excess cost which may result therefrom. C.1.1.5. Contact =96 The CBOC coordinator or designee shall be the main point of contact. The main contact telephone number is at (269) 966-5600, Ext. 5824. C.1.2. Description of Items:=20 Items that may be transported include, but are not limited to: C.2.1. Blood samples - to be transported in a locked container specifically designed for blood transportation. (Copy of the Department of Veterans Affairs medical Center Battle Creek & CBOCs Blood Borne Pathogens Exposure Control Plan)C.2.3. Medical Records - to be transported in a locked mailbag. C.2.4. Mail - to be transported in a locked mailbag. C.3. Qualifications: C.3.1. Offers will be considered only from those offerors who are regularly established in the business called for and who are financially responsible and have the necessary equipment and personnel to furnish services in the volume required for all the items under this contract. Successful offeror shall meet all requirements of Federal, State or City codes regarding operations of this type of service. C.3.2. Each offeror shall submit, with the offer, a letter in duplicate fully describing the make of vehicles, model and year which he/she agrees to furnish under this proposal including the location and telephone numbers of his/her establishment where calls are received and vehicles are immediately available for dispatch. C.3.3. The right is reserved to thoroughly inspect and investigate the establishment, facilities, business reputation and other qualifications of any bidder and to reject any bid, irrespective of price, that shall be administratively determined lacking in any of the essentials necessary to assure acceptable standards of performance. C. 4.0. Drivers/Attendants: C.4.1. A record of each employee as to character and physical capabilities of performing the duties of a courier shall be maintained and made available for inspection upon request. C.4.2. All drivers shall have attained legal age (the age of 21 years) and shall maintain neat, clean and professional appearance. Drivers shall be licensed in accordance with the State of Michigan, to include any special licensure required for the transporting of hazardous waste. C.4.3. Drivers shall not make non-emergent stops, for any reason, during the transport of items under this contract. C.5.0. Vehicle:=20 Vehicles used in the performance of this contract shall be licensed and meet all minimum vehicle requirements as mandated by the State of Michigan, to include Insurance and a Hazardous Waste Certificate if required. C.6.0. Liability: Worker's Compensation and Employer's Liability. Contractor's are required to comply with applicable Federal and State workers' compensation and occupational-disease statues. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy. C.6.1. General Liability: The contracting officer shall require bodily injury liability insurance coverage written on the comprehensive form policy of at least $500,000 per occurrence. C.6.2.=20 Certificates of Insurance shall be received before start of work. C.7.0. Limitations: Under no circumstances shall any person other than the driver ride in a vehicle or handle items being transported. C.8.0. Security: While on VA grounds, contractor shall secure vehicles when left unattended. If vehicle must be left unattended with motor running, a second set of keys will be necessary, so that vehicle can be securely locked. The provision at 52.212-1, Instructions to Offerors =96 Commercial (JUL 2003), applies to this acquisition. The provision at 52.212-2, Evaluation - Commercial Items (JAN 1999), applies to this procurement. Offerors are required to submit the following documentation with proposals: Minimum Requirements: (1) Copy of all insurance certificates specified in C.1.1.2 and C.6.0. (2) Documentation of vehicle and driver licenses required by C.5.0 and C.4.2. Documentation that offeror meets these minimum requirement shall be submitted with offers. Failure to submit the minimum requirements may result in elimination from consideration. Evaluation criteria included below are, in descending order of importance, (1) Past Performance, (2) Company Experience, and (3) Price. Past Performance and Company experience are significantly more important than Price. Offerors are required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items ALT III (JUNE 2003), with its offer. The clause at 52.212-4, Contract Terms and Conditions - Commercial Items (AUG 2000), applies to this acquisition with the following clauses added as addenda to the clause: 52.204-4 Printing/Copying Double Sided on Recycled Paper (JUN 1996), 52.224-1 Privacy Act Notification (APR 1984), 52.224-2 Privacy Act (APR 1984), 852.270-4 Commercial Advertising (NOV 1984), 852.237-70 Contractors Responsibilities (APR 1984), 852.270-1 Representatives of Contracting Officers (APR 1984), 52.217-8 Option to Extend Services (APR 1989), and 52.217-9, Option to extend the term of the contract. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders - Commercial Items (JUNE 2003), applies to this acquisition and the additional FAR clauses cited in the clause are: 52.219-6 Small business set-aside (June 2003), 52.222-21, Prohibition of Segregated Facilities (Feb 1999), 52.222-26, Equal Opportunity (E.O.11246), 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era (29 U.S.C. 793), 52.222-36, Affirmative Action for Workers with Disabilities (29 U.S.C. 793), 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era (38 U.S.C. 4212).
