- This is a combined synopsis/solicitation. The VA Network Business Center, Long Beach, California intends to award a firm fixed price unrestricted contract. The Request for Quote 600-321-03SS entitled "Surgical Table with Accessories" is hereby issued for Commercial items and is prepared in accordance with the format Subpart 12.6 as supplemented by additional information included in this notice and incorporates provisions and clauses in effect through FAC 01-15. The North American Industry Classification Code is 423450 and the small business size standard is considered 100 employees. This BOS announcement constitutes the only solicitation issued. A separate written solicitation document will not be issued. The VA Long Beach Healthcare System, is seeking a contractor to provide a brand name or equal basis, the following equipment: NOTE: ALL THE FOLLOWING EQUIPMENT IS RELATED. One (1) SurgiGraphic 6000 Image Guided Surgical Table; SurgiGraphic Patient Safety Straps; SurgiGraphic Catheter Extension; SurgiGraphic Radial AV Fistula Board (including pad and pair of side rail locks); SurgiGraphic Foot Control; SurgiGraphic Standard Side Rail Adaptor (set of three); and SurgiGraphic Carbon Fiber Extension: as manufactured by Steris Corporation, 5960 Heisley Rd., Mentor, OH 44060. Table configuration includes: Mobile base with self-leveling floor locks; Pendant hand control; Adjustable height, Trendelenburg & lateral tilt functions; True =93earth-plane=94 leveling tabletop; Patient positioning memory (4 positions); Surgeon=92s palm control for 4-way manual float, float lockout, home, height, trendelenburg, & lateral tilt positioning; 4-way manual float top (16=94 longitudinal/8=94 lateral panning); Left-right lateral tilt (longitudinal float capability while in lateral tilt positions); Trendelenburg/reverse trendelenburg tilt (lateral float capability while in trendelenburg tilt positions); 71=BA unobstructed image area (83=94 with carbon fiber extension accessory); 2=94 TLT pressure management pad; 500 pound weight capacity. Proposals shall be submitted on an "all or none basis." Partial proposals shall be rejected. If submitting on a brand name or equal basis, sufficient technical data must be included for the government's evaluation. Delivery terms shall be FOB Destination to the VA Medical Center Loma Linda Warehouse, 11201 Benton Street, Loma Linda CA 92357, with delivery no later than three workdays after contract award. Offeror shall submit a complete copy of provision FAR 52.212-3, Offeror Representation and Certification- Commercial Items (FEB 2000).
