Description
This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-03-T-0078 is issued as a request for quotation. The duties to be performed shall be in accordance with the statement of work, (SOW). STATEMENT OF WORK, RESEARCH TECHNICIAN, OPHTHALMOLOGY DEPARTMENT PRK PROGRAM. 1. SCOPE OF CONTRACT a. The work to be performed is located in the Naval Training Center Branch Medical Center and Naval Medical Center, San Diego. The contractor is a research technician who provides research support for a clinical research program. b. The research technician performs a variety of diagnostic and experimental ophthalmic tests, clerical and administrative duties for the Ophthalmology Department / Refractive Surgery Center in support of ongoing and future research projects c. The research technician is the primary liaison between internal or external work units. d. The research technician provides diagnostic and research assistance to the Chairman, Ophthalmology Department, the RSC Program Director, the RSC Research Director, the RSC Clinic Director, and the RSC Division Officer. e. The research technician is directly responsible to the Refractive Surgery Research Director. 2. DUTY HOURS a. The contractor shall be available for duty at the Naval Medical Center, San Diego and the Naval Training Center, San Diego, CA for a total of eight hours between the times of 0630-1700 (this includes a one-hour lunch), Monday through Friday. The contractor shall agree to extend these hours, as necessary, to ensure experiments are completed or deadlines met. The contractor shall be compensated for these extended hours on a non monetary basis, time off duty of match additional time on duty at the discretion of the Division Officer, Clinic or Research Director. The contractor shall arrive for each scheduled shift in a well-rested condition and shall have had a least six hours of rest from all other duties immediately prior to reporting for shift. 3. DUTIES include but are not limited to : Provides support and assistance to the providers in the Ophthalmology Department/RSC Program by the following: *Obtains the required identifications data and other medical information from patients. *Answers all phone calls in a polite manner which does not elicit complaints *Data collection for research analysis *Receives and responds to inquiries from subjects concerning a variety of medical, personal, and administrative matters; determines the general nature of the inquiry and independently responds to all administrative questions. Responds to similar questions from family members, social workers or outside sources upon instruction and authorization of the professional staff. Alleviate subject concerns by explaining diagnostic procedures and referring specific questions to the doctor. *Assists with tracking down, coordinating, correcting, and submitting CHCS/ADS data for End of Day Reports. *Ensures all clinical areas provide accurate and timely subject visit statistics with 100%compliance, to include entering the correct ICD-9 codes on ADS forms where required and recording the diagnosis of all reports into the clinic diagnosis system. *Maintains currency with changes to the Automated Data Entry systems and maintains adequate logs in the absence of operating computers to keep clinic running smoothly. *Effectively and efficiently triages subjects at the appointment level to appropriate level and area of care following Ophthalmology Department/RSC Program guidelines, appropriately consulting provider for further triage as indicated. *Enters patient data on the clinic database systems using a sophisticated information filing and management system program. *May be assigned to various areas of the RSC clinic as directed by Division Officer. *Reviews and updates subject records for complete information regarding name, rank, social security number, duty status, smoking status, and other items as may be directed prior to patient's visit with the provider. Assembles all requires medical information according to the prescribed sequential order before patient record leaves the Ophthalmology/PRK department. *Ensures subject understands providers instructions and is diagnostic examination. *Performs a variety of diagnostic and experimental ophthalmic tests: - Corneal Topography -Mesoptometry - Stray Light Meter - Pupillometry - Dominate eye determination - Ophthalmic photography - Night Driving Simulator - Contrast Acuity Assessment (CAA) - Visual Research Graphics (VRG) Contrast Sensitivity testing - Low contrast visual acuity testing - Wavefront Analysis - Orbscan - Corneal Endothelial Cell Analysis - Digital Ophthalmic imaging - Ocular Ultrasound - Administers ophthalmic medications *Adheres to infection control guidelines and practice universal precautions. *Performs inventory of assigned examination rooms to ensure adequate supplies are in stock. This shall be coordinated with the supply petty officer to ensure all supply requests are met. *Performs inspection and preparation of instruments, materials, and equipment used to insure that they are clean and in proper working condition. *Attends all annual training classes required by this command. *Assists in presentation of monthly in-service training to non-practitioner members of the clinic and administrative staff on subjects related to eye care. *Participates in weekly and monthly meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist. meeting. *Compiles research results from subject measurement into appropriate databases, and participate in the analysis of the data. *Establishes and maintains appropriate files; maintains correspondence tracking log and tickler file for reports. *Responds to routine and non-technical requests for information readily available from files. This may involve determining whether or not to release the information based on the nature or degree of sensitivity of the information and the requester's authority to receive information. *Assists the reception and scheduling staff as needed: subject check-in, scheduling subject appointments as it applies to research protocols. 4. REQUIREMENTS *An Associate / BA / BS degree in science with relevant laboratory experience or equivalent education and work experience preferable, but not required. In the absence of a degree, certification as a Certified Ophthalmic Technician (COT) or Certified Ophthalmic Assistant (COA) also preferable but not required. *Ability to communicate orally and in writing using grammatically correct English. Knowledge of grammar, spelling, capitalization, and punctuation to process a variety of correspondence accurately. *Knowledge of the mission of Naval Medical Center, San Diego and Ophthalmology Department / RSC Program in relation to the population being served. Knowledge of standard medical and surgical policies and procedures. *Knowledge of correct health record maintenance and filing procedures. Knowledge of procedures for assembly patient charts, filing and recording medical data. *Knowledge of medical terminology and diagnostic nomenclature. Knowledge of basic grammar, spelling (especially medical terminology) and correct punctuation. *Knowledge of how to appropriately respond to subject inquiries and give preparatory instructions to patients for a variety of medical procedures/diagnostic tests required. Knowledge of agency regulations governing patient's eligibility for service. *Knowledge of local forms used for medical record documentation. *Knowledge of the CHCS/ADS systems. *Knowledge of the organizations functions and procedures of the work unit to perform such duties distributed. *Knowledge of the Privacy Act, Patient's Rights, and sensitivity of various information in patient charts. Properly screen requests for sensitive information and obtain approval by proper authority . *Ability to perform ancillary tests listed above (Duties) on RSC Program subjects and familiarity with associated medical / ophthalmological terminology. *Basic knowledge of Navy and Marine Corps rank, military customs and courtesy. *Knowledge of the duties, priorities, commitments, policies and program goals of the staff sufficient to perform assignments such as reviewing publications, directives, and other materials which may effect the RSC Clinic. The contractor shall comply with the medical staff rules, regulation and bylaws related to clinical practice as promulgated by the Medial Treatment Facility Commander and the nationwide standards of practice of the Ophthalmology specialty . The contractor will be neat, clean, will groomed and in appropriated clothing when in patient care areas. Hair shall be neatly trimmed and combed. The contractor shall display an identification badge on the right breast of his or her outer clothing, which includes the contractor's full name and professional status. The contractor shall become acquainted with and obey all station regulations, shall perform in a manner to preclude the waste of utilities, and shall not use Government telephones for personal business. All motor vehicles operated on the installation by the contractor shall be registered with the base security service according to applicable directives. Eating and smoking by the contractor are prohibited in patient care areas and are restricted to designated areas. The contractor is not prohibited, by reason of his or her performance under this contract, form outside employment so long as there is no conflict with the performance of services under this contract. The contractor shall make no use of any Government facilities or other Government property in connection with outside employment. All financial, statistical, personnel, and technical data which is furnished, produced or otherwise available to the contractor during the performance of the contract are considered confidential business information and shall not be used for proposed other than performance of work under this contract nor be released by the contractor without prior written consent of the Department Head or designated representative. Any presentation of any statistical or analytical materials, ore any reports based on information obtained from studies covered by this, will be subject to review and approval by the Department Head or designated representative before publication or dissemination. The Secretary of the Navy has determined that the illegal possession or use of drugs and paraphernalia in a military setting contributes directly to military drug abuse and undermines command efforts to eliminate drug abuse among military personnel. The policy of the Department of the Navy (including the Marine Corps) is to deter and detect drug offenses on military installations. Measures taken to identify drug offenses on military installations and to prevent introduction of illegal drugs and paraphernalia include routine random inspection of personal possessions on entry or exit. If there is probable cause to believe that the contractor has been engaged in use, possession, or trafficking of drugs, the contractor may be detained for a limited period of time until he or she can be removed from the installation or turned over to the local law enforcement personnel having jurisdiction. When illegal drugs are discovered tin the course of an inspection or search or a vehicle operated by a contractor, the contractor and vehicle may be detained for a reasonable period of time necessary to surrender the individual and vehicle to appropriate civil law enforcement personnel. Action may be taken to suspend working privileges as well as installation driving privileges. Implicit with the acceptance of this contract is the agreement by the contractor to comply with all Federal and State laws as well as regulation issued by the commander of the military installation concerning illegal drugs and paraphernalia. 5. FAILURE TO PERFORM a. Should the contractor be unable to perform duties under this contract due to medical or physical disability for more than 13 consecutive days or because of questions concerning competency, ethics, or conduct, performance under this contract may be suspended by the contracting officer until such medical or physical disability is resolved, or until such questions concerning competency, ethics, or conduct have been resolved. If performance under the contract is so suspended, no compensation or reimbursement shall accrue to the contractor so long as performance is suspended. The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-10. Past performance and personnel qualifications based on the SOW when combined are significantly more important than cost or price. Prior to award, the government will review the resumes which includes references for the purpose of ascertaining their qualifications relative to the SOW. Accordingly, the contractor shall furnish such resumes and all professional requirements as stated in the Statement of Work (SOW) to the Purchasing Agent along with their quote. All responsible sources may submit a quote which shall be considered.
