- This is a combined synopsis/solicitation for commercial items prepare= d in accordance with the Federal Acquisition Regulation (FAR) sub part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and THERE WILL BE NO WRITTEN REQUEST FOR QUOTATION ISSUED. The solicitation number for this Request for Quotation is RFQ-246-04-00441. =20 DESCRIPTION: Radiopharmaceuticals delivered to VA Medical Center, Asheville, NC. Orders will be placed orally by authorized medical center personnel with the following terms: 1.=09The quantity may vary based on patient demand and estimates represent quantities used in the past. 2.=09Contractor must provide waste disposal of used radioactive dose syringes, vials, etc.=20 3. Contractor to provide full credit for unused doses. =20 4. Contractor to respond during business hours with preparation and delivery of radiopharmaceuticals within 60 minutes of original notification. =20 5. Contractor shall provide any additional technical support deemed necessary for the technical staff, physicians, patient education, etc. =20 6. Request pricing of individual vials of unreconstituted materials. We may elect not to purchase bulk amounts of Technetium, but rather may elect to purchase individual doses. We may elect not to purchase bulk amounts of Cardiolite, but may elect to purchase individual doses of (1) resting dose 10 mci 99m Technetium Cardiolite, and (2) exercise dose 30 mci 99m Technetium Cardiolite.=20 7. The government intends to award this contract based on =93best value=94 and=20 may split the award if the contracting officer determines this is in the best interest of the government. The government intends to award this contract with a base year (October 1, 2003 through September 30, 2004) and two consecutive option years. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror-Commercial Items. As stated in this clause, the Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror=92s initial offer should contain the offeror=92s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussion if later determined by the Contracting Officer to be necessary. Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and Certification--Commercial Items. An authorized representative of the offeror must sign these representations and certifications. The Offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number) and Tax Identification number. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items.
