- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, F74427315060300, is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 01-07 and Defense DCN 04-26. The solicitation listed here is 100% small business set aside. The associated Standard Industrial Classification (SIC) is 1799. The North American Industry Classification System is 235990 and the associated business size standard is $12 million. If two or more responsive, responsible and technically acceptable small or HUBZone businesses respond, this acquisition will be 100% set-aside for small business, if not, it will be unrestricted. This request for quotation consists of the following: Item 0001: Weekly SPA service & maintenance for fitness center at Travis AFB. Contractor must be within 50 miles of Travis AFB and service the saunas three (3) days a week. A statement of work will be provided with solicitation or by request. Item 0002: Emergency sauna maintenance and repair. Service at Travis AFB, CA shall be from 1 October 2003 to 30 September 2004. The following provisions and or clauses apply to this acquisition. FAR 52.212-4, Contract Terms and Conditions-Commercial Items will be incorporated into any resultant Purchase Order by reference. FAR 52.212-5, Contract Terms and Condition Required to Implement Statues or Executive Orders-Commercial Items include clauses (b)(8), (11), (12), (13), (14), (15), (16), (17), (22) and (c) other that may apply. Vendors must be actively registered with the Central Contractor Registration (CCR). Vendors not actively registered in the CCR will not be considered for an award. The CCR website is http://www.ccr.gov. Contractors are asked to submit their TAX ID number with their proposal. Quotes may be transmitted by e-mail or FAX to (707) 424-2712 and are to be received at the 60th Contracting Squadron no later that 4:30 PST on 21 July 2003. The point of contact for this solicitation is Rheame Griffin, Contract Specialist, (707) 424-7728; email address is rheame.griffin@travis.af.mil. STATEMENT OF WORK SPA SERVICE 1. Description of Services. The contractor shall perform chemical testing/balancing, cleaning, maintenance and repair service for the base spas and saunas located on Travis AFB, CA at building 434 (Base Fitness Center). The services shall be performed in accordance with commercial standards and applicable federal, state and local laws. 1.1 The contractor shall provide a 3-day-a-week (Monday, Wednesday, & Friday) spa service for the men & women’s spas at bldg. 434. If for any reason the contractor is unable to do maintenance on the required days, the contractor shall contact the QAP and the fitness center point of contact 24 hours in advance and shall schedule another day in the same week. The government shall provide an escort at the contractor’s request. 1.2 The contractor shall perform chemical testing from the spa/tub itself and not from the mechanical room. The mechanical rooms shall be kept free of clutter and in order. The contractor shall clean up the area after service/repairs are completed. The contractor shall remove old parts, trash, etc. from the premises. The contractor shall not store more than 5 canisters of excess chemicals in the mechanical area. The contractor shall sign the “Spa Service Log” (Date Serviced, Work Description, Comments & Initials) at the fitness center front desk every day of scheduled service. The contractor shall notify the fitness center point of contact if there are problems with the spa(s) and/or sauna(s). If there are any problems/repairs, the contractor shall notify the fitness center point of contact if there are problems with the spa. In the event emergency repairs are necessary, the contractor shall notify the QAP before purchasing parts and/or commencing service. Contractor shall respond within 16 hours of an emergency call and have a completion time of 48 hours. Contractor shall respond within 24 hours for an urgent call and have a completion time of 5 days. Contractor shall respond within 2 days for routine calls and have a completion time of 2 weeks. 1.3 The maintenance of the saunas shall be on an on-call basis with quarterly service inspections. Any parts or service required shall be brought to the QAP’s attention before purchasing and/or commencing service. Response times shall conform to paragraph 1.2. 2. General Information. The government shall provide access to the contractor’s area of responsibility. One key is provided to the contractor for entry to the spas' mechanical room. The contractor shall immediately report to CE QAP or the Contracting Officer if any occurrences of lost or duplicated keys. In the event a key is lost or duplicated, the contractor may be required to re-key the lock without cost to the government. The contractor shall prohibit the use of keys by any persons other than the contractor's employees. 3. Security Requirements. The contractor shall obtain vehicle registration passes from the visitor control center. The contractor shall obtain personal identification passes for all employees and vehicle passes for all contractor and personal vehicles requiring entry onto Travis AFB. 4. Quality Control Plan. Contractor’s Quality Control must reflect a thorough understanding of the maintenance tasks and proper documentation. Contractor shall practice Quality Control of cleaning, maintenance and repairs of Travis Spas. 5. Quality Assurance Surveillance. Method of Surveillance will be Periodic Review and Customer Complaint, and chemical tests performed 3 times a day by the fitness staff.
