Description
NA Request for Quote Notice: This is a Request for Quote for Occupational Health Physicals. The work will take place in at the Army Aviation Support Facility (AASF) and the Combined Support Maintenance Shop (CSMS), Helena, Montana. The purpose of the Montan a Army National Guard (MTARNG) Mobile Occupational Medical Surveillance Examination Program is to provide job-related medical surveillance examinations to approximately 130 MTARNG full-time support (FTS) personnel who are potentially exposed to health haza rds in the work environment. The contractor will provide all labor, mobile vehicle(s) tools, supervision and supplies to conduct mobile medical surveillance for the MTARNG. This project will consist of a Physical Examination, Audiometric Testing, Vision Screening, Pulmonary Function Test, Laboratory Analysis Testing, Chest X-Ray with B-Reader and Prostate Specific Antigen. The Physical Examination will include Occupation Health History, vital Signs, CBC, CA, and Chem 25. Audiometric Testing will be perf ormed with a range of 500 Hz to 6,000 HZ with a zero reference. The Vision Screening will include Multiphasic vision screening. This shall be performed with the equipment (Titmus type), able to measure the following primary visual skills: a. Visual acui ty for distance and near; b. Lateral phorias for distance and near; c. Vertical phorias for distance and near; d. Depth perception; e. Color perception; and f. Peripheral Vision. Pulmonary Function Testing shall be preformed and will have a PFT. Th is testing will done by a NIOSH certified Pulmonary Function Technician. This testing will include: a. Forced Vital Capacity (FVC) and percent of predicted; b. Forced Expiratory Volume in one second (FEV1) and percent of predicted; and c. Ratio of FVC to FEV1 and percent of predicted. Laboratory Analysis testing shall include the following: a. CBC with differential; b. Blood Chemistry screen 25; c. Urinalysis; and d. Blood Lead and ZPP. The contractor shall conduct Chest X-ray with a PA view and B reade r (qualification of the radiologist). A Prostate Specific Antigen shall need to be tested for. The medical surveillance testing is to be accomplished via mobile unit. Features of the unit should include, but are not limited to, a climate-controlled atm osphere, patient waiting/holding area for registration, testing/phlebotomy stations, and individual audiometric testing booth. They shall be equipped with a separate source/generator to operate in conditions with out access to shore power. Contractor sha ll provide two (2) copies of test results along with a complete listing of results in a ledger form NLT one (1) month after testing has been completed. (Contractor??????s forms are acceptable). In addition, contractor will provide the following computeri zed records, formatted for downloading into MTARNG??????s computerized database (Occupational Health Manager and DOHRS-HC); laboratory, audiometric, and spirometry test results. Results of medical surveillance testing which are questionable and/or critica l clinical significance are to be communicated immediately to the Occupational Health Nurse by telephone. The North American Industry Classification System Code is 62199 with a size standard of $8.5 million. The Request for Quote is anticipated to be ava ilable on or about July 7, 2003. A tentative deadline for the receipt of packages submitted is August 7, 2003. Award is anticipated on or around 11 August 2003 and the services shall be completed No Later Than (NLT) 30 September 2003. This project will be solicited electronically using the electronic bid solicitation system (EBS). THE ONLY WAY TO RECEIVE THE REQUEST FOR QUOTE IS TO REGISTER ON THE USPFO FOR MONTANA WEB SITE, HTTP://WWW.MONTANAGUARD.COM/USPFO_HOME.HTM. Once registered, prospective bidde rs can download the Request for Quote. All requests must indicate business size and minority status. Telephone, fax or written request for a Request for Quote will not be accepted. The Request for Quote mailing list will be available on the USPFO web site. The Government will award a firm fixed-price contract resulting from this Request for Quote (RFQ) to the responsible contractor whose quote conforms to the RFQ, will be most advantageous to the Government, Price, Technical Capability and Past Performance considered. This solicitation is a hundred (100) percent set aside for small business.
