- Management & Training corporation (MTC), Springdale Job Corps Center (SJCC) in Troutdale, Oregon is soliciting hourly rate quotations for the following:(RFQ SJ2004-01) 12 hours per week for Dental Services. SJCC is operated by MTC under contract with The Department of Labor. This is a request for a fixed price quotation as required by MTC at Springdale Job Corps Center. All services are to be performed on site, for approximately 165 students 16-25 years of age. A purchase order contract will be issued as a result of responses to the request for quotation and shall extend from: October 1, 2003 through September 30, 2004. The Contractor will be free to practice his/her profession during the term of the contract awarded, provided, however, that it does not interfere with the performance of services under the contract. MTC?s liability extends to the resulting purchase order. MTC reserves the right to refuse or reject any or all bids at their discretion. A hard copy of the RFQ may be obtained by calling the Contracting Officer. All bids are to be legible, signed and referenced with the RFQ Number on the response. BIDS ARE DUE NO LATER THAN CLOSE OF BUSINESS, FRIDAY, AUGUST 1, 2003 Direct all bids to the attention of the ?Buyer? at the address as follows: Management & Training Corporation -Springdale Job Corps Center-31224 E. The NAICS code for this service is 621210. This acquisition is restricted to small disadvantage, minority, women owned businesses. All responsible sources may submit a quote that will be considered by the Agency. A Firm-Fixed Price purchase order will be awarded using Simplified Acquisition Procedures. The following provisions and clauses apply; 52.214-4, 52.209.6, 52.222-21,52.222-22, 52.222-26, 52.222-35, 52.222-36, 52.222-37 52.224-2, 52.232-18, 52.237-7, and will be used to obtain the best value for the government for this acquisition. In using the best value approach, the contract will be awarded to an Offeror who gives the greatest confidence that they will best satisfy our requirements in an affordable manner. Contract award is estimated to take place August 4,2003. Period of performance will consist of approximately a one-year period If a change occurs in this requirement, only those Offeror?s that respond to this announcement within the required time frame will be provided any changes/amendments and considered for future discussions and/or award.
