Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation is being issued as a Request for Quotations (RFQ). The simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures in FAR Part 12. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-14 and VAAR 97-10. The NAICS is 423450. The nonmanufacturer size standard of 500 employees shall be used for purposes of Government procurement of supplies. Items will be purchased for and delivery will be made to the Stratton VA Medical Center, Albany, NY. Prices should be submitted for the following: Provide upgrades for two existing HP UNIX Workstations (part of the CMS Focus 9200 IIID Radiation Treatment Planning System) to XiO Dual LINUX System. The upgrades should consist of at a minimum: HP XW8000 desk top base system with 3.5 inch floppy drive (1) 10/100Base-T Ethernet connector and keyboard, Xeon Processor _ 2.8 GHz 512kb cache (Primary), Xeon Processor _ 2.8 GHz 512kb cache (Secondary), 2GB ECC DDR-266 module (4x512), 73 GC SCSI Ultra 320 Int. (15k) disk drive, DVD (DVD+R/+RW/CD-RW) Combo, DVIDIA Quadro 4 980 XGL Video Controller, HP 21 Inch High resolution Color Monitor (2048 x 1536) Pixel, External 80/160 GB VXA-2 Tape Drive, Modem-MultiModem with USB Interface-56K, MP-Accelerator, Multi-Threading Dose Calculation Software which has been designed to maximize multi-processor hardware performance (parallel processing), On site installation, Service or Systems Administration, cable _DC-35W/5Parc 3_ SCSI and One Day On-Site (at VA facility) Applications Training Course). Warranty period of one year is required. Unit price is to include shipping, set up installation, training, software, hardware, warranty, and support services necessary to obtain full operational value. The following provisions and clauses apply to this solicitation: 52.212-1, Instructions to Offeror-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders Commercial Items with (b)(1), (15), (16), (17), (18), (19), (21), (30) and (32) checked. FAR 52-211-6 Brand Name or Equal and 52.233-2 Service of Protest, VAAR 852.233-70 Protest Content, 852-233-71 Alternative Protest Procedure and Commercial Advertising also apply to this solicitation. The upgrade offered must meet all the salient characteristics specified above to be considered for award. Quotes will be evaluated on the technical capability and price of the item offered to meet the Government's requirement. Award will be made to the offeror whose product meets the requirements listed above and is most advantageous to the Government. The evaluation criteria are as follows: (1) Must be compatible with the existing CMS Focus 9200 IIID Radiation Treatment Planning System (2) technical capability and (3) price. The Government intends to evaluate the information received and award without discussions. Offers including the pricing information are to be submitted on company letterhead and include the information required in provisions 52.212-1, a completed copy of the provisions 52.212-3, Offeror Representations and Certifications-Commercial Items, and the company DUNS number. Electronic copies of the referenced provisions and clauses are available on the internet at www.arnet.gov/far and www.va.gov/oa&mm/vaar/. Quotations and all required information must be received by 4:00 pm local time June 27, 2003. Quotations shall be submitted to Karen Comiskey, Contracting Officer (500/2-90NCA), VA Medical Center, Albany, NY 12208. Fax quotations are acceptable. Fax Number (518) 626-6849.
