- This is a combined synopsis and solicitation for a commercial item is prepared in accordance with FAR Subpart 12.6., as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation FA2543-03-Q-0004 is a Request for Quote (RFQ). This procurement is being issued as 100% SMALL BUSINESS set-aside. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-13 and Defense Acquisition Circular 91-13. North American Industrial Classification System Code 517310, and size standard 1,500 employees applies to the solicitation. The government intends to award a Firm Fixed Price Purchase Order under Simplified Acquisition Procedures (SAP) using FAR Part 12, Acquisition of Commercial Items. 460 CONS/LGCB, Buckley AFB, CO, has a brand name requirement for purchase of the following items: LINE ITEM 0001: NEW AVAYA 6408D+ TELEPHONE SET; QUANTITY 275--UNIT PRICE: $___________ EXT PRICE: $___________; LINE ITEM 0002: NEW AVAYA 6416D+D TELEPHONE SET; QUANTITY 10--UNIT PRICE: $___________ EXT PRICE: $___________; LINE ITEM 0003: NEW AVAYA 6424D+M TELEPHONE; QUANTITY 50--UNIT PRICE: $___________ EXT PRICE: $___________; FREE ON BOARD POINT (DESTINATION or ORIGIN): __________________; FREIGHT (if applicable): _______________; TOTAL PRICE: $________________; PROPOSED DELIVERY DATE: ______________(Days/Weeks) ARO. (Note: Refurbished, repaired or “gray market” phones are not considered NEW and are not acceptable. Contractor may be asked to provide serial numbers prior to contractor award.) FAR 52.212-2, Evaluation -- Commercial Items, the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government considering best price. Potential contractors must be registered in the Central Contractor Registry, http://www.ccr.gov/ to be eligible for award.
