Description
This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6 and 13.5 as supplements, with the additional information included in this notice. This announcement constitutes the only solicitation. This is a Request For Quotation and a written solicitation will not be issued. Solicitation number RFQ 667-62-03 will be awarded using the simplified acquisition procedures. This solicitation document and incorporated clauses and provisions are those in effect through Federal Acquisition Circular 2001-13. The North American Industry Classification System (NAICS) number is 561320, annual gross receipts $11.5 million. Furnish 1 or 2 full-time equivalent (FTE) board certified/board eligible Gastroenterologists to perform the requirements of this contract approximately 680 hours each. An FTE is defined as a contractor_s employee providing service 8 hours per day, 40 hours per week. The price shall be all inclusive to include travel, lodging, car rental (if necessary), professional liability insurance and all related expenses with providing. THE STATEMENT OF WORK, DESCRIPTION OF DUTIES, REQUIRED CREDENTIALS, SECURITY REQUIREMENTS, SUBMISSION OF QUOTATIONS, SERVICES, QUALIFICATIONS, TERM OF THE CONTRACT, FEDERAL HOLIDAYS, PERSONNEL POLICY, PROOF OF PROFESSIONAL LIABILITY INSURANCE, CONTRACTOR PERFORMANCE MONITORING, PRICING AND METHOD OF AWARD ARE IN THE ATTACHED DOCUMENT. The contractor agrees, in accordance with the terms and conditions stated herein and attached, to furnish the services specified and at the prices specified in this contract. LOCATION OF WORK AND FACILITIES: Gastroenterology Service (Room 7E21), Overton Brooks VA Medical Center, Shreveport, Louisiana 71101. The payments for any leave, including sick leave, holiday pay or vacation time, are the responsibility of the contractor. Authority: This procurement is executed under Enhanced Sharing Authority, Public Law 104-262, Title 38 U.S.C. 8153. PAYMENTS: Sums due for actual work done, the Contractor will be paid monthly, in arrears, upon receipt of a properly prepared invoice in accordance with FAR 52.212-4(g). The Contractor shall submit all invoices to the following address: Chief, Fiscal Service (04), Overton Brooks VA Medical Center, 510 East Stoner Ave., Shreveport, LA 71101. The parties agree that the contractor shall not be considered a VAMC employee for any purpose. A company not in possession of FAR 52.212-3 may contact this office or you may access any clause electronically from the above mentioned internet address. Quotes must be in writing and accompanied by a completed Representations and Certifications and credentialing information and CV for the proposed individual to perform the work or they will be rejected. Facsimile quotes will be accepted. Oral quotes will not be accepted. Award will be made to the offeror whose candidate is most qualified, whose quote conforms to the requirements of the solicitation and has the lowest price, all factors being considered. Award may not necessarily be made to the offeror submitting the lowest bid. Award made be made without discussions. Contracts may be awarded to one, multiple or no vendors.
