PERFORMANCE WORK STATEMENT Modify Exhaust Fan # 7 Building 142 Robins AFB, Ga PROJECT NO. UHHZ 030118 April 22, 2003 1. SCOPE OF WORK: The Contractor shall provide all labor, materials, tools, equipment and transportation necessary to complete, assemble, to Modify Exhaust Fan # EF-7 system and add one (1) Fiberglass Reinforced Plastic (FRP) 28 ? diameter Vertical in-line Mist Eliminator in the Stack out side the existing Exhaust Fan # EF-7 and build a platform around the exhaust Stack at 10? high at building 142, Robins Air Force Base, GA. 2. TASK: A. The intent of this contract is to have a complete operational system in accordance with all applicable Standards and Codes. B. The Contracting Officer at Robins AFB is the only authorized person who can make changes to the contract on behalf of the government. C. Contractor shall conduct tests on all equipments and ensure the systems are functional before the job is accepted. D. Testing shall be scheduled with 778 CES/CEZFB personnel, Poe Mr. Don Huff, Tel: 912-926-2399. These government personnel will be on job site during all testing. E. The Contractor is responsible for disposal of all debrising and comply with all applicable Federal, State of Georgia, any laws and regulations from other states and local where disposal might occur, concerning Environmental Regulations compliance and pollution prevention. F. Contractor shall visit site and determine extent of work required due to existing conditions and new work involved. This extent of work shall constitute the basis for pricing. G. The Contractor is responsible for any damages done to any equipment in Plating Shop by the Contractor?s Personnel during the performance of this contract, and shall be reimbursed the cost to repair such damages to the Government. H. Contractor shall perform the following: 1. Supply and Install one (1) Fiberglass Reinforced Plastic (FRP) 28? diameter Vertical in-line Mist Eliminator in the Stack out side the existing Exhaust Fan # EF-7. Unit will include two (2) tapers to reduce duct size from 38? diameter to accept new inline Mist Eliminator, two (2) spare replacement mesh pad for easy pad change out and serviceability. The Installation will include setting new inline Mist Eliminator, remote location of magnehelic pressure gauge to monitor pad pressure drop and running tubing from inline Mist Eliminator to magnehelic pressure gauge. 2. Supply and Install new Pulleys, Bushings and Belts on the existing Exhaust Fan # EF-7 of KCH make model number NH 44 ?/z to change performance from 22,308 cfm to 12000 cfm capacity 3. Use the existing Dampers to Cap-off the exhaust duct inlets from Tank #s 406, 411, 415, 416, 417 and 424 and Test and Balance on the EF-7 exhaust system for the air flow 12000 cfm 4. Construct a platform of size 10? x 8? around the exhaust stack at 14? high, with grating, ladder and handrails for easy pad removal access. Submit Shop Drawings for approval before contruction. A. ASHRAE - Handbook 1993 Fundamentals; Chapter 33 - Duct Design. B. ASHRAE - Handbook 1983 Equipment; Chapter 1 - Duct Construction. C. ASTM A 90 - Wdght of Coating on Zinc-Coated (Galvanized) Iron or Steel Articles. D. ASTM A 525 - General Requirements for Steel Sheet, Zinc-Coated (Galvanized) by the Hot-Dip Process. E. ASTM A 527 - Steel Sheet, Zinc-Coated (Galvanized) by Hot-Dip Process, Lock Forming Quality. F. Standard Mechanical Code (State of Georgia Edition). G. SMACNA - HVAC Duct Construction Standards - Metal and Flexible, 1985 Edition. H. NFPA 90A - Installation of Air Conditioning & Ventilating Systems. 4. HOURS AND SUPERINTENDENCE: a. Work hours are from 7:30 am to 4:30pm local time, Monday through Friday, except official Government holidays and work curtailment days. b. In the event the Contractor desires to work other hours, obtain prior approval from the Contracting Officer c. If work requires a fire protection system outage, notify the Contracting Officer 14 calendar days prior to the requested outage. The Government will respond within 5 calendar days with an approved outage schedule. Outage shall not exceed beyond normal working hours from 7.30 am to 4.15 pm without prior approval from Contracting Officer. Other than Government furnished material, all material shall be delivered through Gate 4. Vehicles with base decals may enter any gate. d. The contractor shall have a qualified superintendent on-site whenever work or deliveries are to occur. The Superintendent shall be familiar with this contract and shall have the authority to speak for the contractor on all contractual matters. The Government cannot provide equipment, escorts, or tools for any part of this work; nor can Government personnel authorize deliveries. If contractor is not available to accept deliveries when they arrive at Gate 4, the shipment must be returned at contractor?s expense for re-delivery. e. The contractor shall notify the Government of the following events so the Government may coordinate inspections, downtimes, etc.: 1) The Contractor shaH submit to Contracting Officer the list of materials placed on order with anticipated ship date, two (2) weeks after Contract award. All material shall meet the respective Codes and Standards and U.L approved. 2) The Contractor shall submit a written notice to Contracting Officer 14 days prior to commencing demolition work. 3) Installation start date shall be coordinated with Contracting Officer, three (3) days prior to Starting. 4) Submit to Contracting Officer for approval the site plan for lay down area, Office trailer and associated equipment before commencement of work. 5. FLIGHTLINE REQUIREMENTS: Do not perform driving on the flightline without following these procedures: a Prior to driving on the flightline, the contractor shall ensure that all personnel required to operate vehicles on the flightline are trained and certified. The contractor is responsible for scheduling the training through WR-ALC/SE, 926-6271, with adequate lead time so as not to interfere with the contract schedule. Upon completion of the training, an AF Form 483, Certificate of Competency, which certifies that the individual has been trained on flightline driving, will be issued by the Safety Office. This certification must be in the possession of the vehicle operator when driving on the flightline. b. Contractor employees requiring vehicle access to the controlled area of the flightline shall display their company?s name on both the right and left side of the vehicle. Lettering shall be at least one and one-half inches high, and may be permanently- or magnetically affixed to the vehicle. The Government will deny entry to contractor vehicles not displaying the company name. c. Any material suppliers or vendors transporting equipment or material to the flightline shall be escorted by certified contractor personnel. The certified escort can be a passenger or operate a lead vehicle no further than three vehicle lengths in front or better of the escorted vehicle. The contractor shall ensure that all subcontractors comply with all flightline requirements. d. In addition to the validated AF Form 483, vehicle operators shall have a valid state driver?s license in their possession when driving on base. Vehicle operators shall adhere to flightline driving procedures contained in WR-ALC/RAFBI 13-204 and AFR 77-2, Chapter 25. e. Under no circumstances shall contractor employees enter any flightline building or area not covered by the contract. 6. OPERATIONS INVOVLING WELDING, CUTTING, BRAZING OR OPEN FLAME: a. Do not perform Welding, Cutting, Brazing or Open Flame without following these procedures: b. Notify the technical representative of the Contracting Officer (Inspector or Engineer). The Government will then evaluate the need for hot work and, if justified, will issue a Welding and Cutting Safety Checklist. c. Contact the Base Fire Department (926-3487). They will inspect the area, including contractor-furnished fire extinguishers, and issue the burning permit (AF Form 592). d. Observe caution and provide equipment in accordance with NFPA 51B, OSHA 1926.354 , AFOSH Standard 91-5 and EM385-1-1 for safety and health requirement manual. The AF Form 592 must be retained by the foreman, at the work area, for the duration of the work. e. Perform a fire watch to inspect the area and adjacent areas for the evidence of fire for at least one-half hour after completion of the work. Fire watch personnel shall sign the AF Form 592 acknowledging the completion of the fire watch for each occurrence of the work. Return the form to the Fire Department, Bldg 377, one hour after fire watch. f. Welder shall be qualified in accordance with AWS D10.9, Specification for qualification of Welding Procedures and Welders for Piping And Tubing. NOTE 22.
