- Description
- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation is issued as a Request for Proposal (RFP) no. 002-534-3-0012/DB (iii) The solicitation document incorporates provisions and clauses are those in effect through Federal Acquisition Circular 2001-13. (iv) The associated NAICS code for this solicitation is 611620 ?Sports & Recreation Instruction.? (v) Contractor shall provide a series of running and fitness camps/clinics in Navajo communities during the Summer of 2003. (vi) The goals of the running and fitness camps series are uncomplicated. They will provide a fun, safe, and a drug-free environment for the children to experience the joy of physical activity. They will provide successful Indian high school and college role models for the children to identify with, and give them basic skills in stretching, walking, running and nutrition. Goal is to encourage the youth to make a commitment to a healthy lifestyle. Navajo and Native American role models ? especially successful college and high school athletes, should facilitate the camps. Contractor will also hold leadership training in the summer of 2003 for local Navajo and Native American high school runners. One of the goals of the training is to help the runners recognize their potential as role models in their home communities. One way to accomplish this goal is to have running and fitness camps for children. 1. The contractor will be responsible for planning, implementing and evaluating the series of running and fitness camps/clinics in Navajo communities during the summer of 2003, including but not limited to: a.) Contractor to identify all trainers, facilitators, guest speakers and assistants for the camps, as well as making arrangements for their travel/meals. b.) Contractor to hold a leadership training in the summer of 2003 for the local Navajo and Native American high school runners. c.) Contractor to identify locations and secure all required clearances (if any) d.) Contractor to develop and provide all training materials. e.) Contractor to advertise the events/agenda for the camps/clinics. f.) Contractor to coordinate a community fun run event in conjunction with the kids? camp. g.) Contractor to coordinate all efforts with local Navajo Area Indian Health Service ? Health Promotion staff, and the NAIHS Health Promotion program. h.) Contractor to evaluate all camps/clinics including responses from participants and parents. i.) Contractor shall provide clinics large enough and staff well enough to provide a chance for at least 100 students per site to participate. j.) Contractor to hold a total number of clinic days in the summer of 2003 = 28 days k.) Total number of camp days will vary between one-day and two-day camps. 2. The contractor will assist with other activities in support of fitness promotion on the Navajo Nation. Including the development of promotional campaigns, incentives and local running/walking clubs. 3. Reporting Requirements: The contractor shall provide summary reports for each camp/clinic to the Navajo Area Health Promotion program Director, including overall participation rates, summaries of group and individual activities, feedback from students and parents, and recommendations for next year?s programs. (vii) Fitness camps/clinics will be conducted through out the Summer of 2003. (viii) Offeror must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors-Commercial. (ix) The following additional FAR provisions and clauses apply: FAR 52.212-2, Evaluation-Commercial Items, the Government will award a contract resulting from this solicitation to the responsible offeror whose offeror is conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1. Ability to implement community running/fitness camps at the local community level ? (weight of this factor is 25 points.) 2. Ability to provide services on-site in Navajo communities ? (weight of this factor is 15 points) 3. Demonstrated experience in Native American youth leadership training and ability to recruit, train, employ local Navajo and Native American role models and athletes as instructors/facilitators/assistants for community fitness/running camps ? (weight of this factor is 20 points) 4. Methodology and Approach ? (weight of this factor is 20 points) 5. Understanding of the Project ? (weight of this factor is 20 points) (x) FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer. (xi) FAR 52.212-4, Contract Terms and Conditions-Commercial Items, is applicable to the acquisition. (xii) FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, is applicable to the acquisition. (xiii) N/A. (xiv) N/A. (xv) N/A.
