Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation No. TIRSE-03-Q-00017 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-18. The Standard Industrial Code (SIC) is 7629; the applicable size standard is $5,000,000.00. This procurement is set-aside for small businesses. The Internal Revenue Service has a need for maintenance of their fifteen (15) Kiosk machines at the following locations: Waterbury, CT; Wethersfield, CT; Pontiac, MI; Edison, NJ; Cheektowaga, NY; Rochester, NY; Syracuse, NY; Brooklyn, NY; Flushing, NY; Akron, OH; Columbus, OH; Dayton, OH; Cincinnati, OH; Galveston, TX; and Dallas, TX. The Kiosk machines are manufactured by Long Island Technical and consists of the following: laminated plywood exterior, custom pine interior, 6?3? height, 180 lbs., 3? depth and 3? width, a power management subsystem, a fan cooling subsystem, a stereophonic sound subsystem and a custom cabling harness. All components carry the original manufacturer?s ?five year? warranty against defects. The warranties on all of the electrical components are extended an additional year. The contractor shall provide unlimited software support for a period of (12) months, thereafter, at no additional cost to the Government. The software services include the tax content update for current year, annual support service, windows operating system, paradox data base manger, and custom ?Tax Forms and FAQ? software. The contractor shall quote a rate for each Kiosk machine that includes a 12-month extended warranty on the hardware and travel expenses associated with the on-site installation of content updates for 2002 Tax Form Information installed at fifteen (15) locations in January 2003. The hardware for each Kiosk is fitted with a computer, a HP 4050 Laser with Duplexor printer, a 17? touch monitor and a PS2 style keyboard and mouse. All items are warranted for an extended ?one year? period. Service calls; labor and parts are included. Additionally, annual preventative maintenance is performed on all of the equipment. The contractor must repair and ship 2 machines to location authorized by requestor. Services will be required for the period, January 2003 through January 2004 (12 months). An award will be made to the responsible offeror conforming to the solicitation and whose offer represents the best value and is most advantageous to the Government. Description: Quantity: Unit: 1. Service Agreement for fifteen(15) 15 EA Government-owned Kosk machines located at the IRS offices identified in this solicitation. The Service Agreement provides content updates for 2002 Tax Forms Information; and unlimited software support for a period of 12 months thereafter, at no additional cost to the Government. Note: The price should include an extended 12-month warranty on the hardware and travel expenses associated with on-site installation of content updates for 2002 Tax Form Information installed at fifteen(15) IRS sites in January 2003. Reimbursement is subject to the limitations outlined herein entitled, ?Terms and Conditions Applicable to Travel Expenses.? 2. Repair and ship 2 machines from vendor to location authorized by requestor The provision at FAR 52.212-1, Instructions to Offerors- Commercial Items, applies to this acquisition. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications- Commercial Items, with its offer. Any quotes received without the representations and certifications will be considered non-responsive. FAR 52.212-4, Contract Terms and Conditions- Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders?Commercial Items also applies to this acquisition, and the following clauses are applicable: FAR 52.219-8, Utilization of Small Business Concerns; FAR 52.222-21, Prohibition of Segregated Facilities; FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Veterans of the Vietnam Era; FAR 52.222-36. Affirmative Action for Workers with Disabilities; FAR 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era; 52.232-33, Payment by Electronic Funds Transfer?Central Contractor Registration; FAR 52.232-35, Designation Of Office For Government Receipt Of Electronic Funds Transfer Information; IR1052-01-001, Electronic Funds Transfer (EFT) Payments and IR1052-01-002, Paid System. This solicitation incorporates one or more solicitation clauses/provisions by reference, with the same force and effect as is they were given in full text. All FAR clauses/provisions can be read in full text at the website, www.arnet.gov. and information regarding all other clauses will be provided upon request. Quotations are due by 4:00 PM, February 19, 2003.
