- STATEMENT OF WORK BODY PUMP INSTRUCTOR 1. The Contractor shall furnish Body Pump instruction at the Pope Fitness Center, 763 Armistead St., Pope AFB NC. 2. The Contractor is responsible for quality control of completed services to insure the minimum standards of Air Force and Fitness Center directives, rules, and regulations are met. These directives, rules, and regulations are as follows: a. Individuals who use profanity or are disrespectful to a Body Pump instructor will be required to leave the Fitness Center. b. Athletic attire is considered proper attire for any Body Pump class. All attire must worn in good taste; nudity or clothes that contain offensive language will not be worn at any time. The Contracting Officers Representative (COR) will make the final determination on proper attire for class participants. c. The Contractor has its own standards and policies regarding attire and will hold its instructors accountable for proper representation. d. Any dependent with a valid ID card 14 years of age or older may participate in any Body Pump class. Any other children will not be allowed during Body Pump classes. 3. Contractors? responsibilities include, but are not limited to: a. Provide quality instruction to all participants. b. Participants will be required to sign a disclaimer form. The disclaimer forms will be kept on file at the Fitness Center. c. Be knowledgeable in CPR and must hold a current CPR certification. d. Use techniques that are scientifically correct, safe, and appropriate for the individual as determined by the needs assessment. e. Provide a safe environment. f. Monitor all participants as they engage in the exercise and will remove persons who appear to be suffering from extreme exhaustion, heat prostration, etc. g. Maximum class size: 30 participants. Any military or DOD ID card holder is authorized. h. The Contractor will notify the Fitness Center of class cancellations. Unless the situation is of an urgent or compelling nature, the Contractor will give a 48-hour advance notice of class cancellation. i. The following are examples of Government cancellations, contingency exercises, Goals or Productivity Days, Down Days. The Fitness Center will inform the Contractor of these cancellations within 24 hours. The Fitness Center will promptly post all cancellations. The Fitness Center is now open some holidays, and Body Pump classes may be taught during this time period. j. All Government class cancellations, to include contingency exercises, and unscheduled down days, can be rescheduled with the coordination of the Fitness Center. k. Uphold a professional image through conduct and appearance, such as wear Body Pump outfits that are tasteful and with no written profanity. GOVERNMENT FURNISHED PROPERTY AND SERVICES The government shall provide, without cost, the facilities, equipment, materials, and/or services listed below: 1. A current schedule of location dates and time of classes. All classes will be conducted in the Fitness Center. This schedule will be accomplished by the Fitness Center. 2. The Government will provide the Contractor with a schedule of monthly class times and dates at least 1 week prior to the first day of each month. 3. The Government will provide adequate space for instruction. 4. The Government will provide publicity for instruction. 5. The Government will take all registration monies collected for course instruction. CONTRACTOR FURNISHED ITEMS The contractor shall provide their labor, equipment, materials, and transportation necessary to accomplish the work. This includes: 1. The Contractor is responsible for leading and creating a Body Pump routine to include music. Props provided by the Fitness Center may be used by the Contractor. Props may be furnished by the Contractor. 2. Exercise attire that is neat, clean, and present a professional image. 3. Contractor will be responsible for providing the headset, wireless microphone, and stereo equipment. 4. The Contractor is responsible for repairs and replacement unless the repair or replacement is deemed the Government?s fault. 5. The Contractor will be responsible for clean up of area utilized for instruction. 6. Contractor is responsible for all supplies utilized for the course and provides students with a list of required items for the course instruction; i.e. Body Pump weights and bars. CONTRACTOR QUALIFICATIONS 1. Contractor and it?s instructors must have current certification from a nationally recognized institution detailing training received and a statement that he/she is qualified to provide Body Pump instruction. 2. The following are considered nationally recognized institutions: ACE (American Council on Exercise), AFAA (American and Fitness Association of America), ACSM (American College of Sports Medicine). 3. Provide a copy of certification to the COR prior to performing contract requirements. 4. The Contractor and its instructors must be CPR qualified and hold current certification during the contract period. 5. In case of emergencies, the Contractor has the option to find a qualified substitute from an approved list of qualified instructors. In the event an instructor is not found, the Contractor will immediately notify the Fitness Center. 6. The Contractor will provide the Fitness Center with a schedule and instructor name 72 hours prior to the first of the month.
