Description

This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is. This solicitation is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-10and DFARs Change Notice (DCN) 20021220. The North American Industry Classification System (NAICS) Code is 621512. The size standard is $11.5M. This acquisition is 100%, set aside for small businesses. The contract will be awarded based on the availability of funds. The acquisition is for the following services: Routine Diagnostic Images/X-Rays Reading. Contract Line Item Number (CLIN) Line Item 0001, The Contractor shall provide all personnel, labor, equipment, tools, materials, supervision, and transportation or service necessary to perform the following services: Routine Diagnostic Images/X-Rays Reading as defined in the statement of work as follows; Edwards Air Force CA 93524. Period of Performance 1 October 2000 through 30 September 30, 2001. The following provisions and clauses apply: Provision 52.252-1 Solicitation Provisions Incorporated by Reference. This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. Offerors must include a completed copy of the provision 52.212-3, Offeror Representations and Certifications--Commercial Items with the offer. If the offeror does not have a copy of this provision, contact the contract specialist to receive a copy. The following DFARs clauses apply to this acquisition: 252.2127001, Contract Terms and Conditions; and 252.204-7004, Required Central Contractor Registration.
