Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FTSG000203 and this solicitation is issued as a Firm Fixed Price Consignment contract. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-09. The NAICS Code is 421510 and small business size standard is 500. Unicor Fort Worth Texas Sign Factory intends to enter into a Firm Fixed Price consignment contract with 15 days ARO delivery date for the following items: All of these items must meet specifications as noted in specification text in Section B; Line Item 0001 UNICOR Part Number: SHA4120805, .080x48x144, coated, Type 5052, H38 Temper (300 sheets). Line Item 0002 UNICOR Part Number: SHA25100805, .080x30x144, coated, Type 5052, H38 Temper (200 sheets). Line Item 0003 UNICOR Part Number: SHA5120805, .080x60x144, coated, Type 5052, H38 Temper (50 sheets). Delivery shall be FOB Destination of full order amount to UNICOR Fort Worth FPI (UNICOR SIGN SHOP), 3150 Horton Road, Fort Worth, Texas, 76119-5996. At which time the material will be held in consignment for up to 90 days. Payment terms will be net 30 upon UNICOR busting strapped, numbered skids. Remaining inventory will be paid in full at end of 90 day consignment period. The COTR shall be Dean Osborn, telephone 817/413/3105. Provision at FAR 52.212-3, Offer or Representations and Certifications-Commercial Items, must be completed and provided with offer. (IT IS IMPERATIVE THAT THESE FORMS ARE SUBMITTED) FAR 52.211-16 Variation in quantity is zero/zero. FAR 52.219-6 Notice of Total Small Business Set-Aside. FAR 52.212-1, Instructions to Offerors-Commercial Items applies to this acquisition. FAR 52.212-4, Contract Terms and Conditions-Commercial Items apply. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies, which includes FAR 52.203-6. FAR 52.204-6, Contractor Identification Number Data Universal Numbering System (DUNS) applies. The proposals will be evaluated based on past performance 50% and price and price related factors at 50%. FPI9999.999 Business Management Questionnaire must be completed with a minimum of three (3) references of contracts that are of similar size and complexity. FAR 52.232-33 Payment by Electronic Funds Transfer-Central Contractor Registration applies. Vendors shall submit signed and dated proposals to Unicor, Federal Prison Industries, Inc. Attn: Sandy Adams, 3150 Horton Road, Fort Worth, Texas, 76119-5996 (Telephone:817/413/3199) Proposals should be submitted on the SF1449. Proposals and certifications may also be sent by facsimile to Sandy Adams, Contracting Officer (Fax: 817/413/3347). The date and time for receipt of proposals is November 8,2002 at 2:00 p.m. MST. A completed ACH form must be on file prior to contract award. Proposals must indicate solicitation No. description of the item(s) being offered in sufficient detail to evaluate compliance with the requirements in the solicitations, terms of any expressed warranty, price and any discount terms, remit to address, a completed copy of representations and certifications at FAR 52.212-3, and acknowledgment of all amendments, if any. All proposals that fail to furnish required representations and certifications or information or reject the terms and conditions of the solicitation may be excluded from consideration. The Government may make award without discussions with Offerors, therefore, the Offerors initial proposal should contain Offerors best pricing. Any amendments hereby issued to this solicitation shall be synopsized in the same manner as this solicitation and must be acknowledged by each Offeror. Offeror must hold prices firm for 60 calendar days from the date specified for receipt of offers. Proposals or modifications to proposals received at the address specified for the receipt of proposals after the exact time specified may not be considered.
