- This procurement is unrestricted; all responsible sources may submit any offer. This is a combined synopsis/solicitation for commercial services and commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. Solicitation number M67004-03-Q-0013 applies and is issued as a Request for Quote. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-08.This procurement is being sought on an unrestricted basis, it is not a small business set-aside; any responsible source may submit an offer. NAICS 339113, SIC 3842, and Size $6M applies. This is a firm fixed priced effort for six line items as follows. Line Item 0001: Quantity 240 Each. Item Description: Closed Circuit Breathing Apparatus. Equipment shall be portable, rugged, lightweight, and capable of being hand carried. Equipment shall meet all safety requirements identified by NIOSH/MSHA for subject units. Display unit must be easy to read displaying system quick checks, digital and graph pressure availability, LED/Whistle Warning Alarms, time under air monitoring and display. Unit shall provide breathable air for up to four hours (depending on user) at a constant 1.5L/minute O2. Unit must be capable of user preventive maintenance with minimal tools. Unit must be capable of filter replacement, general cleaning, and inspection within minimal time frame. Line Item 0002: Quantity 240 Each. Item Description: Soft Sided Carrying Case designed for use with Line 0001 Closed Circuit Breathing Apparatus. T hese items will be used to transport and store items in Line 0001. Line Item 0003: Quantity 240 Each. Item Description: Reusable Canister designed for use with Line 0001 Closed Circuit Breathing Apparatus. Canisters must be lightweight and meet all safety requirements identified by NIOSH/MSHA for subject equipment. Line Item 0004: Quantity per manufacturer?s specifications. Item Description: Consumables such as filter material, lime, and disinfectant required to support 500 uses of items in Lines 0001 and 0003. Line Item 0005: Quantity: N/A. Item Description: Product Support to include user or level I training for equipment / supplies in Lines 0001-0004. Winning offeror shall provide fit test of masks as part of purchase price. Vendor shall provide a dedicated support agent to the program on a continual basis for two years (24 hours a day and 7 days a week). Ven dor shall provide technical support for life of equipment on a continual basis. Vendor shall have capability (24 hours a day and 7 days a week) to deploy technical and service support to an emergency incident on request for the life of the apparatus. Vendor shall provide emergency transportation for parts on request. Line Item 0006: Quantity N/A. Item Description: Express Warranty. 24 month warranty to include manufacturer parts and labor. Warranty shall include shipment of equipment to and from factory for repair. Warranty shall include shipment of replacement item on a loaner basis (upon request). Delivery of all items is required within 90 days or less after award of contract. Equipment will be delivered FOB Destination to SUPCEN, Attn: Mr. Johnson, 5749 Briar Hill Road, Mail Stop 28, Lexington, KY 40516. The Government will award the contract resulting from thi s combined synopsis/solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government considering technical capability, delivery, and price. The offeror should also provide its Commercial and Government Entity (Cage) code, Contractor Establishment code (DUNS number) and Tax Identification Number. Quotes that are non-compliant with any material requirement of this solicitation may be rejected without furthe r consideration for award. The offeror shall provide its Commercial and Government Entity (CAGE) Code, Contractor Establishment Code and Tax Identification Number. To be eligible for award you must be properly registered in the Government's Central Contractor Registration (CCR). Offerors may obtain information on CCR registration and annual confirmation requirements by calling 1-888-227-2423, or via the Internet @ www.ccr.gov. If a change occurs in this requirement, only those offerors that respond to this announcement within the required time frame will be provided any changes/amendments and considered for future discussions and/or award.
