- The 50th Space Wing, Onizuka AFS, CA, is soliciting offers for maintenance of equipment as specified below. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only occasion for solicitation. Quotes are being requested and an additional written solicitation will not be issued. This solicitation, FA2550-02-Q-0911, is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-09. The North American Industry Classification (NAICS) code is 811219 and the business size maximum is 500 employees. The proposed contract will be awarded as a small business set-aside requirement. ITEM DESCRIPTION: Line Item 0001: Contractor is to provide all tool, parts and labor to maintain Life Fitness Equipment in accordance with the Life Fitness maintenance manual and the attached Statement of Work. This modification is to add the Statement of Work Statement of Work Fitness Equipment Maintenance Contract 1. The Contractor will: a. Provide all tools, parts, and labor necessary to maintain the equipment at the highest performance level as specified in the Life Fitness Maintenance Manual. Preventative maintenance and repairs covered under this contract include the following: - 8 each Life Fitness Lifecycles - 8 each Life Fitness Life step - 8 each Life Fitness Treadmills - 4 each Life Fitness Cross Country Trainers - 1 each Life Fitness Low Back Extension - 1 each Life Fitness Standing Calf - 1 each Life Fitness Leg Curl - 1 each Life Fitness Seated Row - 1 each Life Fitness Rear Deltroid/Pectoral Fly - 1 each Life Fitness Incline Chest Press - 1 each Life Fitness Abdominal - 1 each Life Fitness Leg Extension - 1 each Life Fitness Lat Pull down - 1 each Life Fitness Smith Machine II - 1 each Life Fitness Arm Curl - 1 each Life Fitness Horizontal Leg Press b. Provide quarterly preventative maintenance on all cardiovascular and strength equipment. Parts and labor to be provided at no additional cost. c. Provide unlimited service calls/visits and parts and respond within 3 work days after notification at no additional cost for all cardiovascular equipment (treadmills, steppers, cycles, and cross country trainers) d. Provide quarterly preventative maintenance and one additional service call/visit within 3 work days after notification at no additional cost for all Life Fitness Strength equipment ( Leg Press Machine, Smith Machine, Curl Bench, etc) e. Replace all worn parts with only certified Life Fitness parts at no additional cost. f. Provide Life Fitness logo cloth and leather upholstery were applicable. g. The successful contractor will be required to show proof that the contractor and employee (s) have current certification, and maintain current certification, from Life Fitness to perform maintenance on Life Fitness equipment. Proof will be required prior to award. Ensure that only those employees who have current maintenance certification from Life Fitness equipment are dispatched to Onizuka AFS to perform work on Life Fitness equipment. h. Provide own transportation to and from the work site. Contractor will be required to provide the following information 48 hours prior to visiting the installation.
