- This is a combined synopsis/solicitation for commercial Non-Personal Service Contract prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. Solicitation Number RFP-797-FDF4-02-0053 is issued as a Request for Proposal. This solicitation document and incorporated provisions and clauses are those in effect through FAC97-24. The NAICS Code is 621111. This procurement is 100% Set Aside for small business with preference given to Service-Disabled Veteran owned small Business and Veteran owned small business. The Air Force Medical Logistics Office/Veterans Affairs Special Services (AFMLO/VASS) Contracting Activity has a requirement for a Contractor Health Care Provider (CHCP) the CHCP will serve as the Program Director for Allergy/Immunology at Wilford Hall Medical Center (WHMC), Lackland AFB, TX. The CHCP shall provide care to both Out-Patients and In-Patients who are eligible for care at the Medical Treatment Facility, as authorized by Air Force Instruction (AFI) 41-115. As a minimum, the CHCP must meet the following criteria: MINIMUM QUALIFICATIONS. The CHCP shall have graduated from an accredited school and completed a Internal Medicine or Pediatrics, Allergy/Immunology and Clinical Laboratory Immunology residency acceptable to the U.S. Air Force Surgeon General, and be board certified in Internal Medicine or Pediatrics, Allergy/Immunology and Clinical Laboratory Immunology. The CHCP shall have and maintain a current (within 2 years) Basic Life Support Certification (BLS) and Advanced Cardiac Life Support (ACLS). EXPERIENCE. The provider shall have the following experience levels: Shall have taught in the field of Allergy/Immunology at an accredited allergy/immunology program for a minimum of 10 years. Shall have been a program director at an accredited allergy/immunology program for a minimum of 5 years. The credentials of the CHCP shall be reviewed and privileges granted as outlined in AFI 44-119, Clinical Performance Improvement. Initial applications for clinical privileges, to include credentials action history (any gaps of more than 30 days must be explained), malpractice history, and completed copies of the below listed documents (to include all applicable MTF forms), shall be submitted to the MTF Credentials Office. Copy of valid, ACTIVE, current unrestricted state medical license and applicable Board Certification. Non-personal services contract employees must maintain an active license or authorizing document from the state in which they are practicing. Copies of education and training certificates. =20 Continued medical education for the past 3 years, if applicable (AFI 41-117). Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification (AFI-44-102). The CHCP shall submit three separate references on AF FORM 1562 (Credentials Evaluation of Health Care Practitioners) from current employer or last employer. A signed consent for release of information. Copy of Drug Enforcement Agency (DEA) certificate, if applicable. A list of all states in which contract provider currently holds or has held a license to practice related services. Applications for privileges shall be screened through the National Practitioners Data Bank and Federation of State Medical Boards. Professional staff appointments and clinical privileges are specific to the facility and the provider, to include documents listed above (1.3.1.1.1-1.3.1.1.8); in accordance with AFI 44-119. Initial appointment is for one year and reappointment shall occur and not to exceed two years, in accordance with AFI 44-119. The CHCP shall submit a complete credentials application for processing. Incomplete proposals will be returned without action. If the CHCP has not been granted privileging within 60 days of initial application, all documentation shall be reaccomplished in accordance with MDWI 40-10, Credentialing and Privileging Process.=20 The CHCP shall submit his/her application immediately upon award.=20 Submission of an unacceptable application will be considered a failure to perform. MALPRACTICE HISTORY. If the CHCP has ever been a defendant or the subject of a medical malpractice liability claim, settlement, judicial or administrative adjudication, or any other resolved or open charges of inappropriate, unethical, unprofessional, or substandard care, the contractor shall provide information regarding the action (s). The contractor shall indicate if the action (s) were settled prior to final court action, judgment rendered by a court, defendant found liable, or if the matter is still pending for each action as in accordance with AFI 44-119. DENIAL/TERMINATION OF PRIVILEGES. The MTF reserves the right to terminate the services of the CHCP when it is apparent that their job performance is below accepted standards, their patient management is deemed to be consistently substandard, they develop a record of unprofessional conduct, or when it is in the best interest of the Air Force or its=92 beneficiaries. The following FAR clauses apply to this acquisition.=20 FAR clause 52.212-1, Instructions to offerors =96 Commercial Items; FAR Clause 52.212-4; Contract Terms and Conditions =96 Commercial Items; FAR Clause 52.212-3, Offeror Representations and Certification =96 Commercial Items; and FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders =96 Commercial Items. The government anticipates award of a contract with a base year and four option years resulting from this solicitation to the responsive/responsible offeror whose offer conforms to the solicitation and is most advantageous to the Government.
