Description

NA 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations a re being requested and a written solicitation will not be issued. 2. Solicitation No. DAKF06-02-T-0038. This solicitation is being issued as a ?Request for Quotation (RFQ). 3. The solicitation documents incorporate provisions, and clauses are those in effect through Federal Acquisition Circular 01-09. 4. The proposed contract will be a 100% set-aside for Small Business. For this acquisition, the Small Business Size Regulation is 100, and the North American Industry Classification System (NAICS) is 334510, size standard is 500 employees. 5. List of contract line items number(s) and items, quantities and units of measure: 0001/ITEM: 1 ea; #ABAEA1/OAE, Bio-Logic ABAER/OAE System (includes notebook computer, printer, software, hardware) 0002/ITEM: 1 ea; #RM500CP, Audioscan Real Ear W/color monitor 0003/ITEM: 1 ea; #RE770, Audioscan RE770 Recd Xducer 0004/ITEM: 1 ea; Shipping cost if applicable 6. This requirement allows offerors to submit items that are ?brand name or equal? to the items listed in paragraph 5. See FAR reference at paragraph 13. 7. FOB Destination. Contractor shall deliver to Evans Army Community Hospital, ATTN: Logistic Division, Ft Carson, CO 80913-4604 8. The provisions in Federal Acquisition Regulation (FAR) clause 52.212-1, Instructions to Offerors-Commercial applies to this acquisition to include any addendums to the provision. 9. FAR Clause 52.212-2, Evaluation-Commercial items (1/99) applies to this solicitation. Technical, price and delivery time will be the only factors used for evaluation. Technical and price is equal and significantly more important than delivery time. 10. Offerors shall include a completed copy of the provision at FAR clause 52.212-3, Offeror Representations and Certification-Commercial Items, with their quotation. 11. FAR Clause 52-212-4, Contract Terms and Conditions- Commercial items(5/99) applies to this acquisition. 12.
