Description
This is a combined synopsis/solicitation for commercial items prepare= d in accordance with the format in FAR Subpart 12.6 and Part 13, as supplemented with additional information included in this notice.=20 Simplified acquisition procedures apply and the VA will conduct the procurement according to a combination of Parts 12 and 13. This BOS announcement constitutes the only solicitation issued. A separate written solicitation will not be issued. Request for Quotation (RFQ) 618-31-03 entitled "Blood Cardioplegia Set" is issued under Statutory Authority 41 U.S.C. 253(c)(1) and IAW FAR 6.302 and VAAR 811, the proposed contract is for Brand Name or Equal supplies. The authority is cited and justification made based on the determination that the Government's minimum needs can best be satisfied by Brand Name or Equal supplies. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-08 and VAAR 97-9. NAICS Code 421450 applies and the small business size is 500 employees. Offeror's proposals are hereby being requested in response to requirements under a firm fixed price contract and shall be submitted on offeror's letterhead or bid form and shall contain the name, title, and signature of person authorized to submit proposals on behalf of the contractor.=20 Non sterile Bid Sample plus descriptive literature is required. VA Medical Center, Minneapolis, MN has a requirement for brand name or equal Cobe Cardiovascular, Irvine, CA. Item One - 250 each Blood Cardioplegia BCD Set #BCD VAN8S, Brand Name or Equal. Specifications of item referenced is provided for information: Blood: crystalloid Ratio 8:1, low priming volume, bridged to allow only blood administration, dual crystalloid solution lines and bag spikes, table line separately packaged, and integrated temperature port, bubble trap/air purge, over pressure safety valve, screen filter, and pressure monitor port with separator. Usage is estimated at 21 per month.=20 Proposal to include 3 sets Heart-Lung machine tubing inserts and 3 sets of all necessary hardware. Provide a minimum of one (1) non-sterile bid sample. Contract period will be from October 1, 2002 through September 30, 2003, with one 12-month option to renew for Fiscal Year 2004. Price proposal will include separate price schedules for each Fiscal Year. Vendor will ship on an on-demand, as-needed basis.=20 Deliver FOB Destination. All supplies provided will be Y2K compliant.=20 All responsible sources who can meet the VAMC's requirements and provide items as listed above may respond to this solicitation by submitting a non-sterile bid sample along with price and technical proposals on their company letterhead or bid form. Product demonstrations may be conducted to determine that product offered will meet Government=92s needs. Evaluation factors will include, but may not be limited to, delivery, evaluation of bid sample, past performance and price/price related factors. As evaluations may be subjective in nature, VA staff are solely responsible in determining acceptability of equal products. Products will be delivered either to the VA Medical Center, Warehouse (90D), One Veterans Drive, Minneapolis, MN 55417 or to the Minneapolis VA Medical/Surgical Product Prime Vendor, Allegiance Healthcare. The following provisions and clauses shall apply to this solicitation: 52.252-2 Clauses Incorporated by Reference. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. The Department of Veterans Affairs is currently implementing a Department-wide standardization program. As a result, VA is issuing National Consolidated Contract(s)/BPAs for standardized items. In the future, when any item in this contract is standardized under any National Consolidated Contract or BPA, the item will be deleted from this contract 30 days after written notice from the Contracting Officer. Include a completed copy of 52.212-3 Offeror Representation and Certifications Commercial Items with offer. Proposals should address number of days for delivery after receipt of order; all evaluation factors; names and telephone numbers of contractor personnel for ordering and problems; pricing; and a statement addressing VISA payment acceptance. The Government intends to make award to the offeror that is most advantageous to the Government's criteria. Evaluation factors will include, but not be limited to: evaluation of bid samples provided, descriptive literature, delivery time frame, past performance history, and price.=20 Award will be made to the offeror determined to be the "best value" and may be made with or without discussions. Offers and bid samples are due September 26, 2002, at 2:30 pm.
