Description

This is a combined synopsis/solicitation, for commercial items prepared in accordance with the format of FAR 12.6, as supplemented with additional information in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. Solicitation Number N68096-02-Q-0215 is hereby issued as a Request for Quote (RFQ). Incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-09. The North American Industry Classification System (NAICS) Code for procurement is 421450 and the size standard is 100 employees. The U.S. Naval Hospital, Guam has a requirement for the following, ?Brand Name" Only, Ultrasound System (quantity one (1) each per CLIN Numbers): CLIN 0001. Part #L02464, SonoSite 180 Plus hand-carried ultrasound system, including color; CLIN 0002. Part #P02461, C15e/4-2, microcurved array; CLIN 0003. Part #P02450, ICT/7-4, intracavitary array (biopsy compatible); CLIN 0004. Part #P01114, Sony UP 895 MD video printer; CLIN 0005. Part #P02517, SiteStand mobile docking station; CLIN 0006. Part #P01909, 15? Flat panel display for SiteStand; CLIN 0007. Part #P01370, Sitelink image manager; CLIN 0008. Part #P00552, SiteCharge dual battery charger; CLIN 0009. Part #P00049, Extra battery; with a 12-month standard protection warranty at no additional cost. Delivery terms shall be FOB destination to Defense Logistics Agency, Defense Distribution Depot San Joaquin, Tracy Depot, Bldg. 30 CCP, 25600 Chrisman Road, Tracy, California 95376. Proposals shall be submitted on an all or none basis. Partial proposals shall be rejected. Offeror/s responding to this announcement shall provide Commercial and Government Entity (CAGE) Code, Tax Identification Number and must be properly registered in the Government?s Central Contractor Registration (CCR). This will be a firm fixed price contract and will be awarded utilizing the Simplified Acquisition Procedures, and is solicited using 100% Total Small Business Set Aside.
