- Description
- The Department of Veterans Affairs (VA) Medical Center, San Juan, PR has the requirement for Electrical Surgery Panel including components as shown in CLIN 0001. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR), Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation Number RFQ672-240-02 is issued as a request for quotation (RFQ). The incorporated document and provisions and clauses are those in effect through Federal Acquisition Circular 2001-07. This action is 100% set aside for small businesses. The applicable NAIC is 33531. The panel and all components must be compatible with ?square d? manufacturer specifications. Therefore, the contractor shall meet all of the specific requirements. CLIN 0001 - One (1) Surgical Facility Panel # SDDVP2A506 100 Dual Output Volt Hospital Panel including the following: A. OF 30A GREEN GROUND RECEPTACLES:O 120V POWER RECEPTACLES: 6 20AMP DUPLEX IVORY HOSPITAL GRADE, RECEPTACLE #1: NO RECEPTACLE SELECTED, RECEPTACLE #2:NO RECEPTACLE. B. EIGHT (8) EA.DVBS BACKBOX DUAL VOLTAGE 36 X 64. C. EIGHT (8) EA. DVT522 TRANSFORMER DUAL OUTPUT VOLTAGE PANEL. D. SIXTHEEN (16) EA. ORICA PANEL ISOLATION ACCY-LIGHTS/ALARM. E. SIXTHEEN (16) EA. 1A1C REMOTE INDICATOR ALARM ANNUNCIATOR . F. EIGHT (8) EA. DVC TRIM DUAL VOLTAGE 36 X 64. TOTAL PRICE $_________ Delivery Terms: FOB DESTINATION (DOOR TO DOOR). The contractor shall deliver CLIN 0001 within 30 days or less after contract award to: Department of Veterans Affairs, VA Medical Center, Warehouse loading dock area, #10 Calle Casia, San Juan, P.R. 00921-3201. Inspection and acceptance shall be made at destination. FAR 52.212-1 Instructions to Offerors -- Commercial Items (OCT 2000) is incorporated by reference and applies to this acquisition. The provision at 52.212-2 Evaluation--Commercial Items (JAN 1999) does not apply to this acquisition. Instead, the following information will be used for evaluation of offerors: an award shall be made to that responsible offeror submitting a technically acceptable quotation and offering the lowest evaluated price. Price shall be the deciding factor among technically acceptable quotation. To be technically acceptable, the offeror's technical quotation or proposal must be evaluated as technically acceptable for each of the requirements listed above. Technical acceptability will be determined solely on the content and merit of the information submitted in response to this provision. Therefore, it is incumbent on the offeror to provide sufficient technical literature, documentation, item specification sheets, or brochures, etc., which will provide a complete description of the offeror's panel(s). If the offeror's panel (s) product specification for any of the evaluation parameters is different than what is requested, the offerer must provide a rational as to why and/or how this change will not compromise the overall performance of the panel (s), in order for the Government to make an adequate technical assessment of the proposal. The price proposal shall be evaluated on the overall lowest price to the Government. The offeror shall provide backup information verifying the price offered; e.g. a copy of current catalog, three invoices for the same equipment, etc. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Each offeror shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial (JULY 2002) with its quotation including Standard Form 1449. FAR 52.212-4 Contract Terms and Conditions -- Commercial Items (FEB 2002) is incorporated by reference and applies to this acquisition. Acceptance of the warranty does not waive the Government's rights under the Inspection clause, nor does it limit the Government's rights with regard to the other terms and conditions of this contract. In the event of a conflict the terms and conditions of the contract shall take precedence over the warranty. The warranty period shall begin upon final acceptance of the applicable deliverable listed in the schedule. Quotations must be address attention to Mr. Rey F.
