Description
NA The Department of Veterans Affairs (VA) has an ongoing program to furnish artificial limbs and related services to those veterans that require them and are eligible for assistance. It is VA?s intention to solicit proposals and award multiple contracts on a competitive basis to preferred providers within a VA Medical Center?s (VAMC) jurisdiction under the authority of Title 38 U.S.C. Section 8123. For fabrication and repair of artificial limbs, the prices proposed shall not exceed the established L-Code prices applicable to the geographic area subject to this solicitation and at the time of performance. Facilities that are listed more than once require a contractor for each geographic area described. Please submit a quote on each geographic area for which you want to be considered for award. Offerors must have offices within the one of the counties they are bidding on. It should be noted that repair rates for 15 minute increments are now part of the L-Code pricing schedule. Considering this, please apply the best percentage discount that you offer against the L-Code pricing baseline. (Offers quoting 0% discount may be determined non-responsive and rejected; offerors may quote on more than one facility): GEOGRAPHIC AREA #1: Whatcom, Skagit, Snohomish, Island, and San Juan counties in Washington State. GEOGRAPHIC AREA #2 SET ?ASIDE 100% FOR SMALL BUSINESS: King, Pierce, and Thurston counties in Washington State. GEOGRAPHIC AREA #3: Clallam, Jefferson, Mason, Grays Harbor and Pacific counties in Washington State. GEOGRAPHIC AREA #4: Okanogan, Douglas, Grant, Ferry Lincoln, and Adams counties in Washington State. GEOGRAPHIC AREA #5 SET ASIDE 100% FOR SMALL BUSINESS: Stevens, Spokane, Pend Oreille and Whitman counties in Washington State. GEOGRAPHIC AREA #6: Boundary, Bonner, Kootenai, Benwah, Latah and Shoshone counties in the State of Idaho and Lincoln and Sanders counties in the State of Montana. See Numbered Note #2.
