- This is a Combined Synopsis/Solicitation for commercial items in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number 232-02-0146 is issued as a Request for Quotation (RFQ), under simplified acquisition procedures, as a small business set-aside. All responsible small businesses may submit a proposal. The Government will consider all timely proposals. The NAICS code is 561110 and the small business size standard is $5 Million. The Government intends to make either single or multiple contract awards to the responsible offeror(s) whose offer is evaluated and determined to be the best value for the Government giving consideration to technical merit, price and other factors. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-08. DESCRIPTION: The purpose of this requirement is to acquire the services of a Wellness/Fitness Coordinator and a Wellness/Fitness Associate to staff the General Services Administration?s Ft. Worth Fitness Facility. Contract awards may be made for other Wellness/Fitness Coordinator/Associate positions if Federal Occupational Health Service customer agency needs arise after the issuance of this combined synopsis/solicitation. A complete description of the requirements for this acquisition is available on Fed Biz Opps as Request for Quotation (RFQ) Number 232-02-0146. Proposals will be evaluated based upon the factors as listed below for a base year and four option years. Offerors must review and comply with the FAR provisions or clauses that apply to this solicitation. FAR provisions and clauses are available on the Internet at http://www.arnet.gov/far. The following clauses or provisions are specifically referenced and are required in the response to this solicitation: FAR 52.212-1 Instructions to OfferorsBCommercial Items and FAR 52.212-2 EvaluationBCommercial Items factors include (a) Technical Merit: (1) Mandatory Educational RequirementsBIndividuals must meet this requirement in order to receive further consideration; (2) Training; and (3) Experience. (b) Past Performance; and (c) Price. Technical and past performance, when combined shall be more important than price. Proposal submission instructions are provided in RFQ Number 232-02-0146. Offerors must include a completed copy of FAR 52.212-3 Offeror Representations and CertificationsBCommercial Items in their proposal. The following contract clauses apply to this acquisition: FAR 52.212-4 Contract Terms and ConditionsBCommercial Items; FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive OrdersBCommercial Items with the following paragraph (b) clauses indicated by number (1); (11); (12); (13); (14); (15); (21); and (25). Other FAR Contract Clauses that apply are: 52.217-9; 52.232-18; 52.245-1; and 52.245-2. The deadline for receipt of questions concerning this combined synopsis/solicitation is August 12, 2002 at 2 pm Eastern Standard Time (EST). Questions must be furnished in writing or by e-mail to Naomi A. Haney-Ceresa (nhaney-ceresa@psc.gov). Your questions must be mailed or faxed in sufficient time to be received by the Contracting Officer on or before August 12, 2002 at 2 pm EST. The date and time for submission of proposals is August 26, 2002 at 2 pm. Because of increased security at Federal buildings, please allow extra time for your proposal to be delivered to the Contracting Officer located at: DHHS/Federal Occupational Health Service, 4350 East-West Highway, 3rd Floor, Bethesda, MD 20814, Attention: Naomi A. Haney-Ceresa, Marked RFQ 232-02-0146. Failure to use the specified address could result in your proposal being delivered late. Specific information and instructions for proposal delivery to FOHs East-West Highway office suite is provided in Section E.4. of the RFQ document. All proposals must be submitted in hard copy (original and three copies). Faxed proposals will not be accepted. Offerors must furnish the company name, DUNS number, address, telephone and fax number, e-mail address, if available, and official point of contact in their proposal.
