- Description
- Amendment 0001 for RFP 600-120-02; is to enter into a Capitated Community Based Outpatient Clinic (CBOC) contract for contractor-provided healthcare services for primary care services in the Gardena & South Central Los Angeles CA areas. Also included in this solicitation is an option to award a Capitated CBOC provider services for primary care services at the East Los Angeles CBOC. Greater Los Angeles is requiring that the capitated rate contractor sublease the current VA facilities located at 1251 Redondo Beach Boulevard, 3rd Floor, Gardena, CA. 90247 for the Gardena location and a to determined area for the South Central Los Angeles, CA Community Based Clinic. It is anticipated that these facilities would be providing medical care to approximately 4,000 veteran patients during the first year of operation. The contractor would be required to sublease the VA Gardena CBOC from the Veterans Administration (VA), and provide a turnkey operation in both Gardena and South Central Los Angeles areas. The contractor would be required to sublease the East Los Angeles CBOC located at 55400 East Olympic Boulevard #150, City of Commerce, CA. 90040 and otherwise provide a turnkey operation. There are approximately 1500 veterans currently enrolled at this Clinic. The award of this Clinic is at the option of the VA and can be made at time of award or added to the contract at any time within 90 days written notice to the contractor. The primary geographical area for the South Central Los Angeles Clinic is as follows: West: The US 110 Harbor Freeway, East: The US 710 Long Beach Freeway, North: Slauson Avenue, South: US 105 Freeway. All requests for a solicitation package shall be addressed to the NBC Contracting Officer in writing. Facsimile requests for a solicitation package shall also be accepted at NBC facsimile number (562) 961-1370. The request must contain a Company contact person, telephone and facsimile numbers, and complete address. Telephone request for a solicitation package shall not be accepted. The anticipated solicitation issue date is on or about August 20, 2002 with proposals due on or about September 24, 2002. The North American Industrial Classification System (NAICS) Code is 621498. Companies that have requested a copy of the solicitation package need not reapply again. All other terms of the original announcement for RFP 600-120-02, not mentioned in this amendment will remain unchanged.
