Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 and 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotation being requested and a written solicitation will not be issued. Solicitation Number F0462602T0006 is being issued as unrestricted. This solicitation, Purchase Request Number F7311421720100 is being issued as a Request For Quotation (RFQ).This solicitation document and incorporated provisions and clauses are those in effect through FAC 01-08 and DCN 20020531. The government intends to solicit and award this purchase order for the following: (LINE ITEM 1001: Numeric Pagers Northern California Area coverage, QTY 2650 UI: Months 12 = 31,800) (LINE ITEM 1002: Numeric Pagers, Nationwide Coverage, QTY 50 UI:Months 12 = 600) (LINE ITEM 1003: Alpha Numeric Pagers with paging via internet or web site, Northern California Area coverage, QTY 15 UI:Months 12 = 180) (LINE ITEM 1004: Alpha Numeric Pagers with paging via internet or web site, Nationwide coverage, QTY 20 UI:Months 12 = 240)(LINE ITEM 1005: Group Paging, QTY 18 UI: Months 12 = 216) (LINE ITEM 1006: News and/or information service to any alpha numeric pager, QTY 35 UI: Months 12 = 420) (LINE ITEM 1007: Twenty-four (24) Hour Dispatching to Alpha Numeric Pager QTY 1 UI: Months 12 = 12) (LINE ITEM 1008: Keyboard System for Alpha Numeric Paging QTY 1 UI: Months 12 = 12) (LINE ITEM 1009: Custom Software Package for Alpha Numeric Paging QTY 1 UI: 12 = 12) Description: Period of Performance, Base Period, 1 October 2002 to 30 September 2003, Option Period # 1, 1 October 2003 to 30 September 2004, Option Period # 2, 1 October 2004 to 30 September 2005. The provisions at FAR 52.212-1, 52.212-2 price and past performance shall be evaluated with equal importance. 52.212-3, Offerors must include a completed copy of FAR 52.212-3 Representation and Certifications- Commercial items with this offer. 52.212-4, 52.212-5 (b), (11), (12), (13), (14), (15), (16), 52.217-6, 52.217-8, 52.217-9 (a) 30 days, 60 days; (c) 3 years apply for this acquisition. Payment will be through the Government Purchase Card in accordance with FAR 52.232-36 and subject to Availability of Funds FAR 52.232.18. The Government will make a best value determination with consideration of past performance trade-off. Statement of Work Equipment Specifications, I. DESCRIPTION OF SERVICES: The contractor shall furnish all personnel, equipment, and other items and services necessary to provide leasing of numeric and alphanumeric pagers and paging service to Travis Air Force Base (AFB) in accordance with industry standards and instructions identified by this Statement of Work (SOW). The contractor shall comply with all Federal, State, local laws, executive orders, rules and regulations applicable to its performance under this contract. All pager telephone numbers will be local calls or toll free from Travis AFB. All pagers will operate in the 929 to 932 megahertz (MHZ) band. 1. Numeric paging with unlimited number of calls to pagers. The contractor shall provide Northern California Area coverage including all along the Interstate 80 corridor from Sacramento County to San Francisco County. Coverage will be effective in Fairfield, Vacaville, and all of Travis AFB to include inside all buildings and in accordance with industry standards. The contractor?s pagers will not go into ?out of range? on Travis AFB. 2. Numeric paging with unlimited number of calls to pagers. The contractor shall provide Nation-wide coverage including all major metropolitan areas of the continental United States and as stated in paragraph one. 3. Alphanumeric paging with paging via Internet or web site with unlimited number of calls to the pagers. The contractor shall provide Northern California area coverage including all along the Interstate 80 corridor from Sacramento County to San Francisco County. Coverage will be effective in Fairfield, Vacaville, and all of Travis AFB to include inside all buildings and in accordance with industry standards. The contractor?s pagers will not go into ?out of range? on Travis AFB. 4. Alphanumeric paging with paging via Internet or web site with unlimited number of calls to the pagers. The contractor shall provide Nation-wide coverage including all major metropolitan areas of the continental United States and as stated in paragraph 3. 5. Group paging with unlimited number of calls to the pagers. The contractor shall provide for groups of pagers to be paged simultaneously through one (1) phone number. Paging system is to handle groups up to 1000 pagers. Pagers used for group paging must be able to be a member of multiple groups when requested. Group paging is to be accomplished using a common cap code when requested by the government. Pagers will retain a unique Cap Code for individual paging. 6. News and/or information service on alphanumeric pager. The contractor shall provide the capability to display on the pager. The vendor will supply a list of the services offered. 7. Twenty-Four (24) hour call in to dispatcher with unlimited calls to the dispatcher. The contractor shall provide the capability to contact a dispatcher via telephone whereby the dispatcher then enters the alphanumeric page or pages to be sent to the pager holder. 8. Custom software package for alphanumeric paging. The contractor shall provide a software package to be placed on a computer to allow sending of pages to their alphanumeric pagers. 9. The contractor?s paging system must provide service to Northern California numeric and alphanumeric pagers on all floors of the hospital Building 777. The only exempted areas are those that are lead shielded. A minimum signal strength of ?70dbm on one (1) single frequency in the 929 to 932 MHZ band must be available from the contractor?s paging system on the roof of the base hospital so it can be provided to an antenna located on top of the hospital. All pagers used in and around the hospital will operate on this one frequency. II. SERVICE DELIVERY SUMMARY: The proposal shall include description of the following customer services: Delivery of newly activated pagers, supply of spare pagers, account for reimbursement of lost stolen or missing pagers, system failure notice, correction of billing errors, account representative?s responsiveness, and customer service representative availability. III. GOVERNMENT FURNISHED PROPERTY AND SERVICES: 1. An external roof mounted antenna connected to a programmable paging repeater capable of amplifying the 929 to 932 MHZ band and a radio antenna system inside bldg. 977, the base hospital, to facilitate the distribution of the contractors paging radio frequency (RF) signals inside the hospital. IV. GENERAL INFORMATION: 1. CONDUCTING BUSINESS: The contractor will only conduct business under this contract with the designated representatives identified by the Contracting Officer. The contractor will put in place adequate systems/safeguards to insure all members of their company are aware of and conduct business with only the representatives designated by the government. 2. DELIVERY OF SERVICES: The contractor will deliver newly activated pagers to the government within 7 business days of a request. 3. SPARE PAGERS: The contractor will supply spare, pagers used to exchange for lost or broken pagers to the Personal Wireless Communication Systems (PWCS) office in the 60 Communications Squadron. A minimum of one (1) spare of each type of pager in use at Travis AFB and a sufficient number of the widely used pagers will be provided. The contractor will provide additional spares to this office within five (5) business days of a request for additional spares. 4. REIMBURSEMENT FOR LOST AND DAMAGED PAGERS: A separate account will be created for lost, stolen, or missing pagers. The 60 Communications Squadron will be liable to reimburse the contractor for lost or damaged spare pagers. Individual units or pager holders will be responsible for reimbursement of lost or damaged pagers in their possession. Payments to this account will be via credit card, check, or money order. A price listing for replacement of each type of pager in use by Travis AFB will be submitted to the government. 5. NOTIFICATION OF MAJOR SYSTEM FAILURE: Should the contractor experience a major system failure affecting the Travis AFB area, they will notify the 60 Communications squadron as soon as possible via telephone number provided by the contracting officer. A major system failure is defined as any failure renders all alphanumeric and/or numeric pagers on Travis AFB inoperative. 6. BILLING AND ERRORS: The contractor will correct billing errors within 30 days of notice of the error from the government. A phone number, e-mail address, and the specific office and/or person or persons to contact to correct billing errors will be provided to the government. Multiple accounts will be set up at the discretion of the government. An overall summation of billing for the multiple accounts will also be provided. 7. ACCOUNT REPRESENTATIVE: The contractor will designate an employee to provide support, in person, at Travis AFB for this account. As a minimum this person will come to PWCS office on Travis AFB at least once every 2 weeks. The individual?s responsibilities will include as a minimum delivering new and/or spare pagers, picking up defective pagers, and collecting the checks and/or money orders collected by the PWCS office at Travis for re-imbursement to the company for lost pagers. 8. CUSTOMER SERVICE REPRESENTATIVE: The contractor will designate an employee to serve as the single telephone contact person during the vendors normal business hours for exchanging cap codes on existing pagers, ordering new services, and answering general questions about this account and services the company provides. Proposals in response to this announcement shall be submitted to 60 CONS/LGCB 350 Hangar Ave. Bldg.549 Travis AFB, CA. 94535-2632. All proposals must be received on or before 24 Aug 2002, 4:00 p.m. Any proposals received after the 4:00 p.m. deadline will be determined late. If interested provide pricing and availability.
