- NAICS Code: 561110 The Environmental Protection Agency is seeking a contractor with the ability to conduct a survey of approximately 4,000-5,000 employees agency-wide to gather data on their interest in participating in a child care program. Also, the survey shall include obtaining feedback from the August 2001 Child Care Tuition Assistance Pilot Program participants(15-25 employees). The contractor shall administer the survey to meet a short deadline (August/September 2002). The Survey requirements shall include the following: (1) Electronically (via e-mail/website); or (2) Hard copy via mail. Include pre-stamped, first class envelope; a pre-stamped and addressed return envelope for the ease of respondents; plus (3) an in-depth telephone interview of the August 2001 pilot program participants (15-25 employees). Questions will be developed in concert with the National Program Manager (NPM) for the Child Care Tuition Assistance Program. The scope of work requires that the Contractor choose methods (1) or (2) above, in addition to (3) and in-depth telephone interview of August 2001 participants (15-22 individuals). Survey method electronic or hard copy mail is appropriate for the 4,000-5,000 targeted employees to be surveyed, if the results can be obtained by an agreed upon deadline (August/September 2002 time frame). Deliverables: The Contractor shall work closely with the NPM to: (a) Structure the survey (b) Fine tune questions already developed (5-10 questions) (c) Agree on targeted population for the survey (d) Review survey before final distribution (e) Market survey to targeted population. (f) Tabulate and analyze the results. Reporting Requirements: (a) Present interim reports on survy results as needed. (b) Present a summary report to the NPM by September 30,2002 Data obtained from this survey cannot be used for any solicitations or other purposes other than that specified in the completion of this project. EVALUATION FACTORS FOR AWARD: The Government will make an award to the responsible offeror whose offer conforms to the RFQ and is most advantageous to the Government, cost or price and other factors considered. TECHNICAL EVALUATION CRITERIA: 1. Management and Technical Approach (25 points) Offerors will be evaluated on their approach to planning, organizing, and carrying out contract activities as presented in the Statement Of Work (SOW). 2. Past Performance (25 points). Offerors past performance will be evaluated based on past performance information presented in their proposals and verfied by the Government. 3. Corporate Experience (25 points). Offerors will be evaluated on their current and prior corporate experience managing and administering relevant contracts for design and surveys similar in scope.
