Description
The Federal Bureau of Prisons intends to award a nonpersonal services contract a qualified individual or company to perform Oral Surgery services at the Federal Medical Center, Lexington, Kentucky. Responsibilities include, but are not limited to: The contractor shall provide Oral/Maxillofacial Surgical care on a non-discriminatory basis to include necessary on-site services and/or outpatient services professional services as appropriate which cannot be provided at the FMC.. For reasons of security and cost containment, evaluations and surgical services shall be performed within the confines of the FMC as much as possible. When it is determined by FMC staff that it is medically indicated to transport federal prisoners off the premises for needed medical, surgical or emergency care, prisoners can be transported to community medical facilities. The contractor must make every reasonable effort to provide needed health care services on the premises of FMC, and when this is not practical nor possible, services may be performed in community health care facilities. ON-SITE SERVICES: The contractor will be required to provide the following health care services at the FMC: Oral/Maxillofacial Surgery Services - The contractor will provide diagnostic evaluations, recommend treatment, and perform surgical procedures for patients presented with injury and/or fracture of facial bones, benign or malignant lesions, osteotomies, extraction of impacted teeth, temporal mandibular disorders, apicoectomies, biopsies of oral lesions as well as other diagnoses as requested by FMC's medical and/or dental staff. FMC's dental and surgical team, including anesthesia, shall be available to assist with all procedures. The contractor shall supply the FMC with an adequate number of oral surgeons for those dental specialty areas as required. The estimated on-site sessions shown are not a representation to the contractor that the estimated quantity will be required or ordered, or that conditions affecting requirements will be stable or normal. The FMC will provide the contractor with as much advanced notice as possible for those dental specialties needed and arrange appointments for office examinations. The work schedule will consist of one (1) session per quarter, with session length defined as three (3) hours. The contractor will evaluate approximately 10 - 30 patients per session. This number will decrease if surgical procedures are performed during a session. Guaranteed minimum number of sessions will be two (2) the estimated maximum number of sessions eight (8) for the base year and all option years. Guaranteed minimum number of procedures will be one (1) the estimated maximum number of procedures will be thirty (30) for the base year and all option years. Specific dates and times for the contractor?s work schedules (sessions) will be mutually determined by the contractor and the FMC?s Health Services Administrator. Hours of work must be during normal business days and hours, Monday through Friday, 7:30 a.m. - 4:00 p.m., excluding federal holidays. However, consultation after normal business hours must be available via telephone for emergency situations as needed. OFF-SITE SERVICES (SERVICES PERFORMED AT CONTRACTOR?S FACILITY): The contractor will provide oral surgical services by performing a variety of surgical procedures utilizing appropriately licensed oral surgeons, preferably board certified. Senior oral surgeons may perform surgical procedures under the supervision of a board certified physician. Pre and post-operative surgical consultations will be required, commensurate with community standards. The contractor will be required to participate in coordinating the continuity of care provided to each patient with the patient?s primary physician at the FMC. An operative report must be completed according to community standards and included in the patient?s medical record. The contractor will be required to provide services in a private office setting with a response time no greater than thirty (30) minutes in order to provide emergency dental/surgical care for those incarcerated at the FMC who cannot receive emergency care and/or treatment in-house. Office visits will typically be scheduled during normal business hours on Monday through Friday, excluding federal holidays. However, consultation after normal business hours must be available via telephone for emergency situations as needed. An office visit will consist of one (1) inmate per visit. The estimated visits will consist of two (2) visits per quarter. The guaranteed minimum number of visits will be two (2) and the estimated maximum number of visits will be fifty (50) for the base year and all option years. The estimated visits are not a representation to the contractor that the estimated quantity will be required or ordered, or that conditions affecting requirements will be stable or normal. CONTRACTOR QUALIFICATIONS: The contractor must be a graduate of an approved School of Dentistry with a current unrestricted state license to practice dentistry. The contractor dentists must be a graduate of approved ADA School of Dentistry and completed a residency in Oral/Maxillofacial Surgery with a current unrestricted state license to practice dentistry and surgery, board certified dentists are preferred. The contractor must submit copies of all current medical license for each individual assigned to this contract. The copies must be submitted with the offer. This is a non-personal services contract and not a personnel appointment. Contract period will include a Base Year and four (4) one (1) year option periods, not to exceed 12 months each. This soliciatation is 100% set aside for small business. Hard copies of the solicitation shall not be mailed out. The solicitation shall be made available at this site in approximately two weeks. All responsible sources may submit a quote which may be considered for award. No collect calls.
