Description
REQUEST FOR QUOTATION: Remove 11,990 SQ.FT. of insulation located in bldg 1556 contaminated with bat guano, vacuum, sanitize, wipe down rafters and install new 9? 30-R insulation (roll type). This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. A site visit will be held on 10 July 2002, at 09:00 CDT, attendees must submit: Name, SSN, date of birth, make/model of vehicle, license plate number and drivers license number by 5 July 2002 for entry on to Whiteman AFB, no late requests will be accepted after this date due to command policy established for proper channels being notified for processing of entry authorization letters (EAL). This announcement constitutes the only solicitation under electronic format, quotations are being requested, and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ). Submit only written quotations for RFQ F23606-02-Q-0007. All Statement of work and Evaluation Factors must be met as they appear in this notice. This procurement is 100% set-aside for small business. Reference the Service Contract Act, Wage Determination No: 1994-2307 for Johnson County, MO applies to this acquisition. This solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001.06 and Defense Acquisition Circular 91-13. The NAICS code 235420 applies to this solicitation. The small business size standard is $11.5 million. STATEMENT OF WORK: The job requirements is as follows: 1. GENERAL: 1.1 The Contractor shall provide transportation, labor, materials, equipment, insulation, cleaning supplies, fuel and any other needed material for the purpose of this project. 1.2 Contractor personnel and vehicles must posses appropriate passes. These shall be obtained from Security Forces, Pass and ID, at the Visitor Center, located at the base Spirit Gate. 1.3 The Contractor is subject to all base fire, safety, security and traffic regulations. 1.4 The Government contracts on the understanding that the Contractor is complying with applicable laws concerning workplace safety. 1.5 The Contractor is subject to all citations and penalties for violation of applicable workplace safety laws. The burden of compliance rests with the Contractor. Enforcement of workplace safety laws is the responsibility of the agency specified in the applicable law. 1.6 the Contracting Officer may notify the Contractor of hazards in Contractor operations that endanger Government personnel or property and require appropriate abatement action. This provision does not constitute assumption of liability for Occupational Safety and Health compliance on the part of the Government or negate other agency responsibility for enforcement of applicable laws. 1.7 Accident prevention principles must be thoroughly integrated into all operations. Unsafe conditions and practices, as identified by the Contracting Officer, or designated representative, must be corrected immediately. 1.8 Any and all debris generated by the Contractors operations shall be removed from Whiteman Air Force Base. Disposal of all debris shall be in accordance with existing local, state, and federal regulations. The Contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations. 1.9 The Contractor shall be liable for any loss, destruction, or damage beyond normal wear and tear, and deterioration to Government property, including vegetation and landscaping, resulting from Contractors operations. Any damage to Government or personal property will be repaired or replace to the Contracting Officer?s satisfaction at Contractor?s expense. 2. WORK REQUIREMENTS: 2.1 The Contractor shall remove 12,000 SQ. FT. of batting/roll insulation from the attic space above the third floor ?Enterprise? building 1556. All beam/support structures and walls will be brushed down and wiped off with a disinfectant to remove contamination. All insulation will be double bagged and removed from Whiteman Air Force Base in accordance with paragraph 1.8 above. 2.2 Upon completion of insulation removal, the Contractor shall vacuum sweep the decking and adjacent areas to remove contamination from the surfaces using an industrial HEPA vacuum system. 2.3 The Contractor shall disinfect all contact areas using a bleach and water solution not stronger than a 50/50 mix. The Contractor shall not soak the deck, wall, beam areas in such a manner to penetrate to the ceiling below. The object is to treat by wetting not soaking the areas. 2.4 The contractor shall, after areas are dried, install new 9? R-30 insulation with vapor barrier. 2.5 The contractor will contact the Contracting Officer and the Civil Engineering official for inspection of the area after it?s been sanitized prior to installing the new installation. 2.6 A final inspection will be conducted at the completion of the project. 2.7 Working in the evening is permissible due to extreme heat in the attic during normal duty hours. Contractors may bring fans and lighting tripods for their use. CONTRACT CLAUSES: The provision at FAR 52.212-1, Instructions to Offerors ? Commercial Item, applies to this acquisition.
