- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The reference number is FU044120880300 and the solicitation is issued as a Request for Quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-07. (iv) This is a total small business set-aside. The associated North American Industry Classification System (NAICS) code is 421410 and the small business size standard is 100 employees. (v) This synopsis/solicitation is to replace 8 BARCO projectors with 8 Mitsubishi X400B LCD projectors and the details addressed in the Statement of Work (SOW) for USTRANSCOM at Scott Air Force Base IL. This requirement is a BRAND NAME ONLY. (vi) A site visit is scheduled for Friday, 24 May 2002. Contact MSgt William Henderson, Contracting Officer, at 618-256-9266 for time and location. Site visit is highly recommended. (vii) The solicitation can be viewed at http://www.eps.gov/. (viii) The solicitation package includes the SOW and Department of Labor Wage Determination 94-2309 Rev 25. (ix) The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instructions to Offerors, Commercial Items, applies to this acquisition. Addenda to 52.212-1(b)(10) Past Performance Information--Provide past/present performance reference. Only references for same or similar type service desired. Past Performance Surveys: The Government will evaluate the quality and extent of offeror's experience deemed relevant to the requirements of the RFQ. The Government will use information submitted by the offeror and other sources such as other Federal Government and commercial sources to assess experience. Provide a list of at least three (3) but no more than five (5), of the most relevant contracts performed for Federal agencies and commercial customers within the last three (3) years. The offeror shall furnish the following information for each contract referenced. Company/Division Name; Product/Service; Contracting Agency; Contract Number; Contract Dollar Value; Period of Performance; and Name, Address, Fax and Telephone Number of a Point of Contact. (x) The FAR provision 52-212-2, Evaluation, Commercial Items, applies to this acquisition. Paragraph (a) is completed as follows: (1) Technical Approach, (2) Past Performance, (3) Accelerated Performance, and (4) Price. Evaluation factors (1), (2), and (3) when combined are significantly more important than price. The Government reserves the right to award to other than low bidder based on the offer that represents the Best Value to the Government. The offerors proposal must contain the (1) Technical Approach--Integration with existing system; (2) Past Performance--Referenced in (ix) above; and (3) Accelerated Performance--Milestone showing proposed delivery and installation for each location contained in the SOW. (xi) A completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications, Commercial Items, MUST be submitted with the quote. The provision may be copied from website http://farsite.hill.af.mil/VFFARA.HTM. Quotes WILL NOT be considered without the completed provision. (xii) The clause at FAR 52.212-4, Contract Terms and Conditions, Commercial Items, applies to this acquisition. (xiii) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition. The clauses at subparagraph (b) (11), (b) (12), (b) (13), (b) (14), (b) (15), (b) (16), (b) (17) and (b) (22) of 52.212-5 are applicable. (xiv) The clause at Defense Federal Acquisition Regulation (DFAR) 252.204-7004, Required Central Contractor Registration applies to this acquisition. (xv) All responsible sources may submit a quotation, which shall be considered by the Agency. (xvi) Contract award cannot be made to any contractor who is not enrolled in the Department of Defense (DoD) Central Contractor Registration (CCR) program. The CCR website is www.ccr.gov/.
