Description
The provision of Occupational Therapy Services is required under P.L. 105-17, Individuals with Disabilities Education Act of 1997. Services consist of evaluation of referred students in the area of occupational therapy, a written report reflecting the findings of the evaluation, participation in a multidisciplinary team meeting regarding the evaluation results, development of goals and objectives for those students identified as needing occupational therapy services, and provision of occupational therapy services for identified students. Services will be provided to eight (8) different BIA schools located within a 50-mile radius of Shiprock, New Mexico on the Navajo Nation. Services must be provided in accordance with P.L. 105-17 and the State of New Mexico special education guidelines. Some services can be provided through the utilization of para-professionals employed at the school site, having been provided work direction by the contractor. Services will be provided for the 2002-2003 school year, starting August 19, 2002 and ending May 23, 2003 (37 weeks), with four (4) option years. Services will be provided at the school sites and the agency education office for a total of 40 hours per week. Realizing this position is difficult to fill; proposals are encouraged for individual schools or groups of schools. For instance a contractor may not be able to fulfill a request for 5 days of service per week at eight different school locations, but could accommodate a request for services at two of the schools for two days per week. Partial proposals will be accepted and partial proposals are encouraged. The solicitation will be issued no later than April 27, 2002. All interested sources may request a copy of the solicitation via email, at dgutierrez@lab.bia.edu, fax or mail. All responsible sources may submit a proposal, which will be considered. All correspondence should include the solicitation number, your full name/company name, address and phone and fax numbers. Requests or questions by telephone are strongly discouraged. NOTE: THIS NOTICE WAS NOT POSTED TO FEDBIZOPPS.GOV ON THE DATE INDICATED IN THE NOTICE ITSELF (05-APR-2002). IT ACTUALLY FIRST APPEARED ON THE FEDBIZOPPS SYSTEM ON 08-MAY-2002.
