Description

NA This modification is hereby issued to ADD the following statements: Submit quotes on the Request for Quotation form (SF-18) and fax to (808) 438-8588 no later than the closing date and time. SAFETY REQUIREMENTS. The Contractor is required to submit an Accident Prevention Program Administrative Plan (POD Form 248-R (rev) 1Mar95) and an Activity Hazard Analysis (POD Form 184-R (Rev) 22Apr91) in accordance with the EM 385-1-1. Forms shall be sub mitted to Mr. Rudolfo Atoigue or Mr. Steve Paahana of the Honolulu Engineer District, Logistics Management Office. Contractor shall not perform work before the safety form are accepted by the Safety Office. Original Description: Procurement of carpet. COLOR, 6589 Bluestone; WIDTH, 12? Broadloom and Modular Tile; PILE CONTENT, Colorstand Infinity Nylon; TILE SIZE, 17 5/8? X 17 5/8?; WARRANTY, 20 year performance warranty. Service includes, remove existing car peting and pad off the floor. Note that existing floor tiles in Room 123 contain asbestos. Contractor shall take appropriate abatement measures not to disturb existing ACM and provide an Industrial Hygienist, air monitoring, and disposal manifest for debr is-containing ACM. Properly dispose of old carpet. Install new adhesive type of carpet tiles without padding, cove base, baseboard moldings, and gripper edge to hold carpet in place. Total Square Footage is 765. Two separate system furniture (L-shaped and T-Shaped Partitions) required to be lifted for the removal of old carpeting and to install tile carpet. Performance period is not to exceed Three (3) weeks after date of award. Work is to be performed weekends only, 0800-1500 hours.
