Description

The original description of this announcement is changed to read as follows: The Bureau of Reclamation, Grand Coulee Power Office, Grand Coulee, Washington, has a requirement for medical services for its employees. The services are to include: emergency treatment of on-the-job injuries for a maximum of 2 visits per occurrence; limited treatment for non-job related injuries or illness with a limit of 1 visit per occurrence; and referrals to physicians. Emergency treatment to be available 24 hours a day, 7 days a week with a Health Care Unit to be available from 9:30 a.m. to 11:30 a.m. Monday through Friday each week. Immunizations for common diseases, influenza, and tetanus will be available to all employees. Some physician ordered services will be administered: allergy shots, vitamins, etc. The facility must be located within 15 minutes travel time from the Power Office. The Government intends to negotiate on a sole source basis with Coulee Community Hospital, Grand Coulee, Washington in accordance with FAR 6.302-1. The services provided under this program must be available within a reasonable commuting distance (approx. 15 minutes travel time, one way); offered on a 24 hour per day, 7 day per week basis; offered through a facility staffed with the needed medical expertise to properly administer services under the program. Coulee Community Hospital is the only organization in the commuting area of the Grand Coulee Power Office, Grand Coulee, Washington, that meet these criteria. If you submit a proposal in response to this notice you must demonstrate your capability to perform the services requested at a fair and reasonable price. The period of performance shall be one year beginning approximately October 1, 2002 through September 30, 2003, with four option years to renew. If you are interested in obtaining a copy of this solicitation, please submit your request in writing to: Bureau of Reclamation, Attn: PN-3704, 1150 N. Curtis Rd., Boise, ID 83706 as soon as possible, but in any case no later than June 25, 2002. The applicable North American Industry Classification System (NAICS) Code is 622110 and the size standard is $ 29 million. This requirement is NOT SET-ASIDE. See Numbered Notes 22 and 26.
