Description
NA CONTRACT SPECIFICATIONS: Medical surveillance testing to begin 1 May 2002 and be completed not later than 31 May 2002.Quotes shall be made on a per test basis. All numbers are approximate with expectation of at least 80% participation of employees in the program. The Contractor s hall be responsible for any special equipment needed to perform services. Proposed schedule for testing shall be submitted prior to start of work for review and approval. Testing will only be performed Tuesday through Friday from 0715 to 1700 each day. Aut horization by the State Occupational Health shall accompany each employee at time of testing. Contractor to provide all hard copies of test results and a complete master listing of results in ledger form NLT 45 days after all testing has been completed. . Contractor shall provide each Employee directly a copy of all test results along with physician comments within 2 weeks of testing. Physician to review all test results for accuracy, and shall report any possibly serious abnormal results as soon as reason able but not more than 1 week from time of test to the Occupational Health Nurse. Physician to perform on site, hands-on examination of employees that are on the respirator program Physician examinations may be completed at the time of the van testing or t he physician may set up a separate visit to the site and must be completed before 14 June 2002. One consolidated invoice for all sites shall be submitted NLT 30 days after completion of tests. Contractor shall provide all test results on a disk that is com patible with the Army National Guard Occupational Health Monitoring System. EXPLANATION OF REQUESTED SERVICES: Tests to be administered by certified operator only. CLIN 0001AA: AUDIOGRAM Approximate Quantity: 382 Audiogram with range of 500 Hz to 6,000 Hz with a zero reference. Calculations should be without age correction. CLIN 0001AB: VISION SCREENING Approximate Quantity: 382 Muliphasic vision screening equipment (Titmus type), able to measure the following primary visual skills: Visual acuity for distance and near. Lateral phorias for distance and near. Vertical phorias for distance and near. Depth Perception.Color Perception.Peripheral Vision. CLIN 00001AC: BLOOD CHEMISTRY PROFILE Approximate Quantity: 329 To include but not limited to at least the following: (1) Comprehensive Liver function profile (2) Cholesterol with HDL to total cholesterol ratio. CLIN 00001AD: BLOOD CHEMISTRY (LEAD AND ZZP) Approximate Quantity: 10 Analysis for lead levels and analysis for zinc protoporhyrin levels. CLIN 0001AB: PULMONARY FUNCTION TEST WITH CLEARANCE Approximate Quantity: 64 Including as a minimum the determination of the forced expiratory volume in one second (FEVI) and the forced vital capacity (FVC). Written clearance by a physician to wear a respirator. Test to be administered by certified operator only. CLIN 0001AF: Physical Examination Approximate Quantity: 64 IAW AR 40-5 and AR 385-10, The parameters for the examination shall be set by the Physician. The examination shall be sufficient enough to give written clearance for the wear of a respirator.
