- Description
- This is a combined synopsis/solicitation issued for commercial items and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. This procurement is being conducted under FAR Subpart 13.5 test program; simplified acquisition procedures apply and VA will conduct the procurement according to a combination of Parts 12 and 13. This announcement for Request for Quotation RFQ 618-117-02 entitled Brand Name Only SARNS 8000 Perfusion System hereby incorporates provisions and clauses in effect through FAR FAC 2001-05 and VAAR 97-8. NAICS Code 339112 applies for this procurement and the small business size standard is 500 employees. No separate written solicitation document will be issued. Contractor's proposals are hereby being requested in response to requirements listed below and shall be submitted on contractor's letterhead and must address all requirements listed. Contractor's response should contain the name, title and signature of person authorized to submit proposals/bids on behalf of the contractor. Descriptive literature for each item offered may be included in proposal/bid package. VA Medical Center (VAMC), Minneapolis, MN has a requirement for purchase of Brand Name Only SARNS 8000 Perfusion System, Terumo Cardiovascular Systems Corporation, Ann Arbor, MI. Brand Name Only is justified IAW FAR 6.302-1. Brand Name Only equipment is described as follows: SARNS 8000 Perfusion System consisting of 1 ea 15401 Base 5 Unit 115V; 5 ea 16402 Roller Pump 115V; 1 ea 16404 Safety Monitor; 1 ea 5773 Air Sensor 3/8x3/32; 1 ea 195240 Level Sensor II Pads 60; 1 ea Cardioplegia Monitor; 16413 Arterial Monitor; 1 ea 16421 Monitor Mounting Arm; 1 ea 16422 Battery Module 115V; 1 ea 16420 Lamp System 8000; 1 ea 164235 Sechrist Blender; 1 ea 16417 Communications Module; 1 ea CCPro Perfusion Data Software; 2 ea CCPro Perfusion Software License; 2 sets each of Operation and Service Data Manuals; operator and Biomedical Instrumentation Service personnel training; installation and warranty for parts and labor; Y2K Compliance warranty. Delivery shall be FOB Destination (unless vendor states otherwise) to VA Medical Center, Warehouse (90D), One Veterans Drive, Minneapolis, MN 55417. Award will be firm-fixed price; payment may be made by Government Purchase Card (VISA). Evaluations may be conducted to determine that the product(s) offered meet Government's requirements, technical capability and past performance history, delivery time frame and warranty to determine "best value" to the Government. Evaluation and/or award may be made without discussions. All responsible sources that can meet the requirements and provide items as listed above may respond to this solicitation by submitting cost and technical proposals on their company letterhead or bid form.
