- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The solicitation number is RFP-RMAST-02-049. The solicitation is issued as a request for quote (RFQ) for a Mental Health Program at the Pineridge Job Corps Center located in Chadron, Nebraska. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-01. This synopsis/solicitation is a total small business set aside. The associated NAICS code is 621330. The small business size standard is $5 million. FAR 52.219-6 Notice of Total Small Business Set-Aside FAR 52.228-5 Insurance-Work on a Government Installation (JAN 1997) AGAR 452.228-71 Insurance Coverage, Alternate I (NOV 1996) Pursuant to FAR clause 52.228-5, Insurance-Work on a Government Installation, the Contractor will be required to present evidence to show, as a minimum, the amounts of insurance coverage indicated below: (a) Workers Compensation and Employer's Liability. The Contractor is required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit worker's compensation to be written by private carriers. (b) General Liability. (1) The Contractor shall have bodily injury liability coverage written on a comprehensive form of policy of at least $500,000 per occurrence. (2) The Contractor shall have property damage liability insurance shall be required in the amount of $500,000 per occurrence. (c) Automobile Liability. The Contractor shall have automobile liability insurance written on a comprehensive form of policy. The policy shall provide for bodily injury and property damage liability covering the operation of all automobiles used in connection with performing the contract. Policies covering automobiles operated in the United States shall provide coverage of at least $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage or loss. SPECIFICATIONS - MENTAL HEALTH DIVISION 100 - GENERAL SPECIFICATIONS 110 - Scope of Contract - The service consists of providing the planning, development and administration of a mental health program for Pine Ridge Job Corps Civilian Conservation Center, Chadron, Nebraska. 120 - Location and Description - the work will be performed primarily at the Pine Ridge Job Corps Civilian Conservation Center, located eleven (11) miles south of Chadron, Nebraska, on Highway 385. 130 - Government-Furnished Property and Services - The Government shall supply to the Contractor, materials, supplies, property or services (hereinafter referred to as "Government-furnished property") as necessary. The Contractor shall be liable for all loss or damage of such government-furnished property until completion and final acceptance of work required under this contract. The Government shall provide staff-developed agenda items periodically drawn from personal experiences, problems and conditions, plus other pertinent ideas. 140 - Contractor-Furnished Property and Services - Contractor shall furnish all necessary training materials, expertise and coordination to implement staff-oriented training based on staff furnished problems and ideas. The contractor will be responsible for the inclusion of outside presenters--authorities in the topic being presented--to supplement other training materials used. The contractor must have prior approval from the Contracting Officer's Representative for any "outside presenter" being considered for staff-oriented training. 150 - Service Schedule - Proposed service schedule shall furnish a minimum of seven (7) hours weekly on Center by the Center Mental Health Consultant. Three hours (3) of this time shall be devoted to staff training, three (3) hours shall be reserved for student evaluation and one (1) hour shall be allotted for review of prospective new student folders. Services by the Center Mental Health Consultant and the contract Psychiatrist will be provided on Thursday. The actual working schedule will be coordinated with the Contracting Officer's Representative. Time spent overall must equal a minimum of seven (7) hours weekly on Center. In addition, six (6) hours of service twice a month will be provided by a licensed psychiatrist to monitor the progress of all students on psychotropic medications. Staffing with the Center Counselors by the psychiatrist and mental health consultant is required on each scheduled visit. 160 - Qualifications - The Mental Health professional shall be fully licensed, certified or accredited by the State of Nebraska as a professional psychiatrist, psychologist, or psychiatric social worker. A COPY OF LICENSURE, CERTIFICATION OR ACCREDITATION AND RESUME OF QUALIFICATIONS MUST BE INCLUDED WITH THE RETURN SOLICITATION PACKAGE. If the applicant is not licensed in the specialities listed above, allowance must be made for ongoing supervision by someone who is licensed as a psychiatrist, psychologist, or psychiatric social worker. The Mental Health professional shall possess the following qualities and skills: 1. Capabilities to establish good rapport with staff and students. 2. Ability to perform staff training, consultation and direct student treatment. 3. Ability to assist the Center in planning, developing and administering a Mental Health Program. 4. Understanding of specific needs of the Center population. DIVISION 200 - TECHNICAL SPECIFICATIONS 210 - Performance Technology - The services to be provided will include, but will not be limited to, those mental health services specified in the Job Corps Policy and Requirements Handbook, Chapter 5: Health Services, 5.1 (R3) Mental Health Program and Technical Assistance Guide 6-D and 6-L and other Job Corps directives. The services to be provided shall include: 1. The development, implementation and monitoring of the total mental health program: a. Advice to the Center Director, Contracting Officer's Representative and health services staff on all mental health matters. This shall include recommending student terminations for mental health reasons and the cost of mental health services including inpatient facility use. b. Staff development in using mental health principles and techniques. This shall include coping with problems of substance misuse, sexuality, aggressive behavior and poor interpersonal communications. c. Consultation on the design of a staff development program to promote the coordination and integration of mental health with other center programs and activities, including counseling, residential living, education and recreation. This shall be done in cooperation with the Center Physician, Center Dentist and other health and counseling professionals, as well as with other staff members involved with students. d. Consultation with Center health staff, counselors, residential living advisors, instructors and other appropriate staff concerning the ongoing mental health problems of students. e. Assistance with program development for the purpose of meeting the life crisis' of students in constructive ways. For example: involvement with the Center disciplinary program, the maximum benefits program, the intergroup relations program and the Zero Tolerance to Drugs and Violence policies. 2. Direct Services to Students on Center as follows: a. Assurance of the maintenance, confidentiality and safeguarding of all necessary mental health records. Whenever the Health Professional evaluates a student, he/she shall enter a brief note in the medical record containing the professional's finding, assessment or diagnosis and plan. b. Issuance and maintenance of Standing Orders regarding mental health matters as prescribed in the Policy and Requirements Handbook, Chapter 5: Health Services 5.3 (R9). c. Diagnostic and evaluative services to students who are inpatients or outpatients. d. Emergency services to students as deemed necessary by center Health and/or Counseling staff on an individual request basis, and appropriate referral for students who must be terminated for mental health reasons. e. Provide psychotropic drug management, monitoring and limited forms of psychiatric treatment with prior approval of the C.O.R. 1. If this service is not directly available from the contractor, the contractor shall make arrangements to sub-contract this service. The contractor shall make available to the C.O.R. for prior approval, the names and qualifications of any sub-contractors. Signed approval from the C.O.R. will be required on any sub-contractors for this service. This service shall be provided at the Pine Ridge Job Corps Center. 2. Notification to parents of minor students shall be made by the Contractor when psychotropic medications are prescribed. Minor students' psychotropic medications may not begin without parental consent. In the event the parent refuses recommended medication, the student may be medically separated from the program. The professional shall adhere to the policies, procedures and guidelines established by the Job Corps. The professional and the Pine Ridge Job Corps Center further agree that the professional shall have the primary responsibility under the Contracting Officer's guidance to implement the program outlined above. The professional, with the concurrence of the Center Director or Contracting Officer's Representative, may delegate by written and dated Standing Orders, any of his/her responsibilities set forth in this contract to appropriate licensed staff. The professional shall be assisted in the development, implementation and operation of the Center Mental Health Program by the Contracting Officer's Representative who will act under guidance and direction from the Medical Director, Job Corps, Washington, D.C., or his/her delegate. 240 - Emergency Care and Consultation - Emergency care shall be provided at the medical clinic/hospital evenings, weekends, or when the Mental Health Professional is not available. Emergency service must be available in the immediate area--within a driving time distance of 30 minutes or less after notification. Separate records and billing shall be kept and submitted. 250 - Coverage - Contractor shall identify licensed mental health consultants by whom coverage may be provided in instances where the contractor is unable to provide emergency or other required services considered critical to the program. Contractor shall provide names and phone numbers in writing to the Contracting Officer's Representative for approval and shall keep the list updated quarterly. Arrangements for coverage will require prior approval of the Contracting Officer's Representative. Charges for the coverage services shall be comparable to the charges for services under this contract. If charges exceed the terms of the contract, the Contractor shall be responsible for the additional charge. 260 - Referrals - In the case of recommendation of medical separation, the Contractor and Sub-Contractor are responsible for written documentation, notification of parents, and referrals to appropriate follow-up treatment. The aforementioned is required by the Contractor/Sub-Contractor on the day of recommendation for medical separation, prior to departure from Center. DIVISION 300 - INSPECTION 310 - The Government will perform periodic inspections of the total contract work, including records maintenance. Job Corps consultants may be included in these reviews and inspections. DIVISION 400 - Payments 410 - Payment will be made monthly upon receipt of proper, itemized invoice at the contract unit prices for services performed and accepted. The Contracting Officer's Representative will receive an itemized statement of services rendered by the 5th of the month following the month the services were rendered. Emergency services shall be itemized on a separate monthly invoice, showing patient's name, date and services rendered. Unit prices shall be the same as shown in the contract for these services. Psychotropic drug management monitoring services shall be itemized on a separate monthly invoice, showing patient's name, date and services rendered. Unit prices shall be the same as shown in the contract for these services.
