- This is a combined synopsis solicitation for commercial items prepared in accordance with the format of FAR Part 12.6 supplemented with additional information contained in this notice. This procurement is being conducted using Simplified Acquisition Procedures in accordance with FAR Part 13. This announcement constitutes a formal request for proposal (RFP) and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-04. RFP NTSBR0200007 should be referenced on all proposals. The NAICS Code is 811213 with a small business size standard of $6.0 million The National Transportation Safety Board (NTSB) 490 L?Enfant Plaza East, SW, Washington, DC 20594, uses the Lucent Technology ISDN Definity G3si telephone switch and Intuity Voice Mail Systems for its daily telecommunications. The NTSB requires the technical assistance of a certified technician to provide telephone set relocations, additions and changes, repairs and troubleshooting system shutdowns. The Technician must have attended and successfully completed the Lucent Technology Training Center for the Definity Switch and Intuity Audix Voice Mail System. The technician must be on site one day a week for 6 hours. In addition, the contractor must: 1. Provide parts for multimedia and analog line cable installations. e.g. power supplies, phone cords, cable for TV connections; 2. Be familiar with old cross connect wiring blocks in telephone wire closets; 3. Be able to remove obsolete telephone closet equipment without damaging existing wiring installations and equipment; 4. Troubleshoot and repair minor and major (total system shut down) telephone system and voicemail outages some of which may be the result of the service providers; 5. Be accessible by pager or cell phone and respond to requests for service within 24 hours of notification; 6. Respond to major emergency outages (system completely shut down) within 4 hours of notification. Submit with your proposal: 1. Names and qualifications of technicians to be assigned to the resultant contract. Technicians must have attended and successfully completed the Lucent Technology Training Center for the Definity Switch and Intuity Audix Voice mail system. Proof of successful completion is required. 2. At least three but not more than five Companies or Agencies for which your company has provided similar services. Include the name and phone number of the person in the company that is familiar with your company?s performance under the contract. 3. The price proposal should include prices for a base year and two option years for each of the following: (a) Hourly or daily rate for on site service 6 hours per day one day a week. This rate may include parking or any other cost associated with providing the required services. (b) Rate for on call service with 24-hour response (c) Rate for 4-hour response for major system outage If you bill for parts or supplies furnished at other than your cost include the basis of how you will bill for these items. Offerors may propose additional response times with associated rates provided they have proposed rates for the response times set forth above. The services and contract requirements may be changed by written contract modification. The modification shall be prepared and approved by a NTSB Contracting Officer. The contractor?s employee shall report to the contracting officer?s technical representative (COTR) upon arrival. The COTR will have accumulated work requests from NTSB employees during the preceding week. These will be prioritized and provided to the contractor upon arrival. The Contractor shall comply with all applicable Federal and local laws, Executive Orders, rules and regulations applicable to its performance under the resultant contract. The following clauses and provisions apply to this acquisition and may be obtained from the web site: 52.212-1 Instructions to Offerors ? Commercial Items; the offeror shall submit a completed copy of provisions at 52.212-3 Offeror Representations and Certifications ? Commercial Items with their proposal. The resultant contract will include FAR 52.212-5, and incorporate by reference FAR 52.212-4. The following clauses will be included in an addendum: FAR 52.217-8 Option to extend services and 52.217-9 Option to extend the term of the contract. Proposals are due 4:00 p.m. local time (EST) Tuesday April 2, 2002. An official authorized to bind the offeror?s organization shall sign all offers. All proposals shall be submitted to the Contracting Officer, NTSB, 490 L?Enfant Plaza East, SW, Washington, DC, 20594. If hand delivered they shall be taken to the receptionist on the 6th floor. Propopsals that meet the stated requirements will be further evaluated for past performance and price.
