Description
This is a solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Request for Quotations are being requested and a written solicitation will not be issued. Solicitation number F7380120520200 is a Request for Quotation. This solicitation document and incorporated provisions are those in affect through the Federal Acquisition Circular 97-27. This procurement is 100% set-aside for small business concerns. The North American Industry Classification System Code is (561110). The small business size standard is $5 million. The basic contract period will be 1 Apr 02 ? 30 Sept 02 with four option years. The offeror shall provide as a response to this solicitation: (1) a resume detailing relevant training and experience; (2) a list of at least two recent references; and (3) a complete price list (Attachment 1), and (4) a completed clause 52.212-3. (Attachment 2). Proposals may be submitted in hard copy or electronically through e-mail or mail. Regardless of method of delivery, it is the offeror?s responsibility to ensure receipt of the proposal by the time and date stated herein. STATEMENT OF WORK for Elementary Faith Formation Coordinator (St. John Neumann Catholic Community) Charleston AFB Chapel a. Environment: 1. Charleston AFB is a Continental United States (CONUS) military Community with a split population of Active Duty, Reserve, Civil Service employees, retirees, and dependents. The Charleston Chapel program has the responsibility of meeting the needs of this diverse community in its support of Charleston?s continuous global mission. 2. The Charleston AFB Chapel is composed of Chaplains, Chaplin service support personnel (CSSP), appropriated and non-appropriated fund contractors, and lay volunteers. The Programming developed by this staff is implemented in a pluralistic environment and is subject to the standards established by the US Air Force. 3. The Chapel community is a faith Group supporting programs at Charleston AFB. This community operates an energetic ministry that requires a high degree of coordination and cooperation among its staff. B. CHARACERISTICS OF THE ELEMENTARY R.E. PROGRAM: The Religious Education Coordinator?s ministry is established to provide leadership in the design and implementation of programs for youth and children to assist in building a solid foundation for spirituality, Christian living, and services through reflection upon life experience in the light of Jesus? Gospel message. The Catholic Elementary Religious Education Program at Charleston will encompass the needs of Children from preschool through 8th grade. The coordinator is responsible for overall development programming and implementation, as well as recruitment and training of staff. C. REQUIREMENTS FOR AN ELEMENTARY R.E. COORDINATOR: 1. Qualifications- a. Need: A positive, motivated, and self-directed individual with the breadth and depth of knowledge necessary to accomplish the day-to-day coordination and administration of the Elementary R.E. Program. The individual is to be an active member of the Catholic faith who has acquired training and experience in religious education, teacher training, theology and scripture, and practice, as articulated by the Second Vatican Council (and subsequent statements of the American National Conference of Catholic Bishops). Applicants will be evaluated based on a submitted resume and references, as well as the results of an interview to be conducted by the Wig Chaplain and/or designate(s). b. Character: An ability to work unsupervised in a fluid and sometimes-stressful environment, alongside staff, adults, and children. Possess and ability to maintain healthy, functional interpersonal relationships with colleagues and constituents. Maintain professional standards of conduct and public decorum including (but not limited to) dress and grooming. The Contractor will be neatly attired and well groomed. Shirts must have collars and worn as designed. Cut-offs, blue jeans, or shorts are not acceptable for males. Female attire will not consist of jeans, cut-offs, pedal pushers, halter-tops, and similar attire. Any hair length, beards, or mustaches are acceptable, provided it is neat, clean and properly trimmed. Contractor must conduct himself of herself in professional and business-like manner. Must have transportation and local telephone. All personnel dealing with youth are subject to a positive background check. 2. Skills- a. Professional: Must be able to efficiently organize and manage diverse program elements. b. Communication: Must be able to communicate the needs and goals of pastoral program to chapel community. c. Computer: Must be fluent with, or capable of learning word-processing, database management and accounting programs in a Microsoft Windows setting. D. TASKS TO BE PERFORMED: 1. Attends weekly Catholic Staff and Chapel Staff meetings as directed by the Catholic Chaplin or his designate. Attends the Pastoral Council meeting (as a non-voting member). Submits to the Catholic Pastoral Coordinator a program status report for the Pastoral Council no later than 5 working days prior to the council meeting. 2. Develops and tracks, in agreement with the Pastor and Pastoral Council, the St. John Neumann Catholic Community High School Faith Formation budget. The proposed budget must be developed and submitted to the Pastoral Council during the April Planning session. 3. Develops a proposed 12-month Calendar of events and scheduled classes with the Pastor and Pastoral Council to be submitted to the Wing Chaplain for approval no later than 1 May. 4. Maintains the required AFI 1270 database and submits monthly report to the Pastoral Coordinator by the 1st working day of the following month. 5. Conducts ongoing registration and maintains a database of students and staff. Updated lists are to be submitted quarterly to the Pastoral Coordinator. 6. Recruits and adequately trains volunteer?s to staff the Religious Education Program. 7. Supports the logistic need of the teaching staff in a timely manner. Maintains adequate supplies of consumable materials. Inventories resources and equipment and submits a report to the Pastoral Coordinator by 1 May. The Coordinator is responsible for proper scheduling and securing of facilities, arranging transportation needs of program, submitting fund requests in accordance with local policy. 8. Maintains a continuity file for the Religious Education Program and after-action reports. 9. Provides weekly publicity copy for all activities to the Pastoral Coordinator in accordance with local policy. 10. Collects and deposits fees for program requirements, fundraisers, and activities according to HCOI 32-2. 11. Prepare, coordinates, and transmits sacramental records involving the Religious Education Program to the Pastoral Coordinator as required?12. Provides an on-going religious Education Program on a weekly basis as approved or determined by the Catholic Chaplain or his designate. 13. Annually develops Parent Handbook of policies, procedures and class calendar as well as sacramental guidelines. Military Archdiocesan requirements, and norms. The handbooks should be distributed at registration. Provide teachers handbook to include job descriptions, annual calendar, policies and procedures as well as sacramental norms. The teacher handbook is to be provided to teachers? two weeks prior o the beginning of classes. The Catholic Chaplin must approve both handbooks prior to their distribution. 14. Hours of work not to exceed 10 hours per week. E. TIME AND PLACE OF PERFORMANCE: 1. Time: Task Dependent; Contractor must be available to coordinate with chapel staff and volunteers. 2. Place: Charleston AFB Chapel [#1 or 2], and various designated locations for classes. (The Chapel will provide adequate work and storage space, office supplies, and repro. Contractor provides own transportation.) The provision at FAR 52.212-1 - Instructions to Offerors ? Commercial Items is applicable to this solicitation. The provisions at FAR 52.212-2 - Evaluation ? Commercial Items is applicable to the solicitation and blanks in (a) are completed as follows: price, technical capability, and past performance, with technical capability and past performance when combined being greater in importance than price. Offeror shall include with their offers a completed copy of the provision at FAR 52.212-3 - Offeror Representation and Certifications ? Commercial Items. FAR clauses 52.212-4 - Contract Terms and Conditions ? Commercial Items, 52.253-1- Computer Generated Forms, 52.232-18 - Availability of Funds, and 52.219-6 - Total Small Business Set-Aside are applicable to this solicitation.
