- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR, Subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ) number 626-02-16. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 2001-04. The NAICS code for this solicitation is 339112. The small business size standard is 500 employees. Furnish, deliver and install a Dornier Compact Delta Lithotripter or equal with the following components: (1) Therapy Unit with electromagnetic shock wave source - maximum penetration depth of 150mm, six variable energy levels from 7.1 to 47 mJ, energy flux density from 0.1 _ 0.25 mJ/mm2, pressure in focal area 210-556 bar, focal size 7.7 X 81mm, Shock wave release: fix frequency of 50-120 shock waves per minute, or ECG triggering, positioning of shock wave source: 40 degrees lateral variable, motorized 3 axis shock wave positioning system, flexible, motorized coupling of therapy head with separate moveable c-arm, 0-25 degrees for ureteral stones, 25-50 degrees for kidney stones; (2) Integrated X-ray locating system with control panel and monitor _ high frequency x-ray generator, fixed anode, double focus x-ray tube, 9_ image intensifier, TV chain with CCD camera, 17_ fluoroscopy monitor, fluoroscopy kV-range: 40-110 kV, fluoroscopy mA-range: 0.2-4 mA, Hf fluoroscopy kV-range: 40-110 kV, Hf fluoroscopy mA range: 0.5-6.5 mA, Automatic self correcting exposures (ABS), 4-image memory storage w/second 17_ monitor; (3) Video Printer, Sony UP-980MD or equal; (4) Handheld control Panel; (5) Urokit for urologic procedures other than ESWL, consisting of leg supports, flexible band drain assembly, twenty ct. drain bags, IV pole, elbow rest, radiation protection, IV arm support, head protection, foot switch; (5) Patient Table, floating table, 120V transportable version with movement range of height (Z axis) 180mm, length (X axis) 90mm, Lateral (Y axis) 100mm, TKCD _10 degrees to _190 degrees, TKCF _8 degrees to + 45 degrees, hand held control unit, transportable gurney, foot operated brakes, table height 820mm-1120mm, table top dimensions 660mm W X 2160mm L.. In addition, the lithotripter shall have the following characteristics: (1) The shock head shall move 200 degrees, under to over the patient to position Ureteral stones and large patients; (2) shall have less that 2% variation in shock wave peak pressure per shock; (3) shall have 15 cm focal depth; (4) Patient table shall be motorized and capable of treating patients up to 400 lbs. and (5) shall have the move and shoot capabilities to simultaneously release shock waves while adjusting the position of the therapy head to improve targeting and speed treatment. Offer shall include de-installation and removal of existing lithotripter for trade-in, installation of new system, restoration of existing space due to de-installation and installation of equipment, application training, and biomedical training for 2 technicians to include airfare, ground transportation, meals, and lodging.
