- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request For Quote (RFQ), solicitation number F05611-02-T-0030, in accordance with FAR Parts 12 & 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-03. The North American Industrial Classification System (NAICS) number is 488991 and the business size standard is $5M. The proposed purchase order is reserved for small businesses. Women owned and disabled veteran owned small businesses are encouraged to submit quotes. USAFA DORMITORY MOVE PROJECT ? The project consists of 96 rooms, to include 93 sleeping rooms, one dorm office, one computer office and one day room. The required items to be moved and/or stored for a sleeping room consists of the following: double headboard, mattress, box spring, metal frame, nightstand and lamp with shade, recliner chair, entertainment center (three drawer with two open shelves), five drawer dresser, and floor lamp with shade. The project shall be completed in four phases. PHASE I consists of removing the contents of thirty-six sleeping rooms, twelve per floor-(three floors). The contents will be removed off-site into storage and stored for approximately six months. Double mattresses shall be packed in mattress cartons for safety and in accordance with Department of Defense MTMC regulations. PHASE II consists of moving twenty-one sleeping rooms into newly remodeled rooms within the same dorm building, and moving one dormitory office into a new room. PHASE II consists of moving thirty-six sleeping rooms into newly remodeled rooms within the same dormitory building. PHASE IV consists of relocating the thirty-six rooms from storage to remaining 36 dorm room spaces. Work to begin on or about 18 February 2002. LINE ITEM 0001: Total Dormitory Move and Storage $__________. The clause at 52.212-1 Instructions to Offerors-Commercial Items with the following addenda applies to this solicitation. FAR 52.212-1(f) In addition, quotes may be faxed between the hours of 7:30 A.M to 4:30 P.M., Monday thru Friday, to Operational Contracting Office, Attention: Kelvin Delaney, Fax (719) 333-4404. Please call (719) 333-4761 prior to faxing your quote. Quotes must be received by the receipt time and day specified at the end of this notice. 52-212-1(e) Multiple offers are not acceptable. Offers shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications--Commercial Items, with its offer. The clause 52.212-4, Contract Terms and Conditions--Commercial Items, with the following addenda applies to this acquisition: 52.212-4(i) Payment-The contractor may submit monthly invoices for incremental payments to the paying office and shall also forward a copy to 10CES/CEPV, 8120 Edgerton Dr., Suite 40, USAF Academy, CO 80840, FAX #719-333-3337 for certification of services received and subsequent submission to the paying office as a receiving report. 52.228-5 Insurance-Work On Government Installation, to include Workers Compensation and employer liability coverage of at least $100,000.00; bodily injury liability insurance coverage written on the comprehensive form of policy of at least $500,000.00 per occurrence; and automobile liability coverage of at least $200,000.00 per person and $500,000.00 per occurrence for bodily injury and $20,000.00 per occurrence for property damage; 53.232-33, Mandatory Information For Electronic Funds Transfer Payment; 52.237-2 Protection of Government Buildings, Equipment, and Vegetation; and 52.242-15 Stop Work Order. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items applies to this acquisition. Additional FAR clauses cited in the clause applicable to this acquisition are: (b)12, 13, 14, 15, (c) 1, Service Contract Act ,Wage Determination 1994-2079, Revision 19; (c)2, Statement of Equivalent Rates for Federal Hires, Employee Class 21130, Shipping/Packer, WG-04, $9.41. DFARS 252.212-7001, Contract terms and conditions required to implement status of Executive Orders applicable to Defense Acquisitions of Commercial Items, NOV 1995 applies to this acquisition with the inclusion of the following DFARS: 252.204-7004 (MAR 1980)?Required Central Contractor Registration.
