- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) subpart 12.6, as supplemented with additional information included in this notice and following the simplified acquisition procedures in FAR part 13.5. This announcement constitutes the only solicitation: proposals are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number 6TSA-02-JMT-0073 is being issued as a request for proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-14. This procurement is for the following: CLIN 1: Geographic, REASON ? Software Site License, QTY. 1; CLIN 2: Two, two day, Analyst-Level Root Cause training held on-site at Norfolk Naval Shipyard facilities. The classes can hold no more than twenty-five (25); CLIN 3: One, Three-day Advanced-Level class ? This class can hold no more than twenty-five (25) students. The Analyst-level course is a prerequisite. CLIN 4: Maintenance package. As well as maintaining REASON? License, the Corporate Maintenance package includes free REASON? upgrades and ten (10) hours of Consultant and/or Technical assistance via phone, fax or email annually; CLIN 5: Up to eight (8), two-hour, REASON? Orientations, These are to be scheduled two per day simultaneously during Analyst-level trainings. Up to 400 students oriented. Please break out quote to show (a) One time costs associated with Implementation, (b) Recurring Costs to Support Program, i.e. License/Maintenance Fee; (c) Optional Recurring Costs, i.e. additional training. This requirement is for the Norfolk Naval Shipyard in Norfolk, VA. Please include all requirements on quotations. No partial quotes. No substitutions. Indicate FOB destination on quote (shipping included in price). The provision at FAR 52.212-1, Instructions to Offerors, applies to this acquisition. Evaluation is based on best value including past performance, cost and ability to meet service requirements. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at FAR 52.212-4, Contract terms and Conditions-Commercial Items, applies to this acquisition. In addition to the guidance contained in FAR 52.212-4, contractors shall submit proof of delivery with their invoices. Proof of delivery may be in the form of a bill of lading or other shipping document signed by the receiving office. Invoices received without proof of delivery will not be accepted. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
