Description

NA Subject Request for Quote is for commercial items and is being processed in accordance with the format in Subpart 12.6. It is being issued as an RFQ using Simplified Acquisition Procedures (SAP) authorized in Subpart 13.5. Madigan Army Medical Center, Ta coma WA intends to establish a firm fixed-price standing order for services that will provide and install 300 Category 5 Local Area Network drops for the period beginning on or about 01 February 2002 through 31 Jan 2003. Performance of service will be in accordance with the latest revisions of all governing codes, standards of industry and the Madigan Army Medical Center Facilities Management Division. The Request for Quote number is DADA13-02-T-0017; the NAICS code is 541519 with a size standard of $18,0 00,000.00. For the purpose of establishing an official record, requests for copies of the RFQ shall be submitted in writing, or electronically via e-mail to angela.chaplinski@nw.amedd.army.mil or facsimile 253-968-4922. Copies of the RFQ will be availabl e on or about 14 January 2002. The request should include business name, point of contact name, complete address, telephone number, RFQ number, business size, tax identification number, cage code and Dunn and Bradstreet number. Telephonic requests will n ot be honored. Firms quoting should ensure registration in the Central Contractor Registration Database prior to submission for consideration. This procurement is 100% small business set-aside.
