Description
The VA Nebraska Western Iowa Health Care System, (VA NWIHCS), Omaha Division, 4101 Woolworth Avenue, Omaha, NE 68105 is using full and open competition for a Community Based Outpatient Clinic (CBOC) at Norfolk, NE to provide health care services for eligible VA referred beneficiaries. Contractor's clinic facility must be physicially located within the city limits of Norfolk, NE. The Omaha Division, VA NWIHCS will serve as the parent facility for the CBOC. Contractor shall provide all necessary board certified physicians and staffing to provide health care services in accordance with JCAHO standards. Payment to contractor will be on a capitated per month per referral basis, for the period commencing approximately February 1, 2002 through January 31, 2003, and 4 subsequent renewal periods. VA referrals will include eligible VA beneficiares residing within the counties of Antelope, Boone, Boyd, Cedar, Colfax, Cuming, Dakota, Dixon, Garfield, Greeley, Holt, Knox, Madison, Nance, Pierce, Platte, Stanton, Thurston, Valley, Wayne and/or Wheeler counties in Nebraska. The VA currently has 300 +/- veterans enrolled at the clinic who will automatically be enrolled with the successful contractor. The contractor will provide all personnel, clinic facilities, equipment, supplies and supervision for the health care services. The contractor will be responsible for all radiological and laboratory services associated with providing primary health care services. The contractor must have a primary care provider on call 24 hours per day, 365 days a year to respond to enrolled patient telephone inquiries. The VA will make an award to the offeror which represents the best value fo the VA. Contract award will be based upon the evaluation of offers using selection factors including price as a selection factor. RFP No. 636-05-02 has been assigned to this solicitation. NAICS code 62111, Offices of Physicians, with a small business standard of $7.5 million per annum will be used. This solicitation is unrestricted.
