- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ), number R3-WZ-02-15. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. NAICS code 333120 applies and small business size standard is 750 employees. ITEMS: LINE ITEM (1) 40 Foot Belly Dump Trailer Tandem-Axle. Capacity: 20 Cubic Yards single hopper. Dimensions: Length 40 feet, Height 9 feet 1 inch without bang boards, Width 96 inches. Gates: Commercial grade steel with chain stops. Air controlled: electric controlled from cab of truck and manual from the ground. Top of Doors T-1 steel. Hopper: 54 inches by 108 inches opening, double gate with three-sixteenth inch plate with one-quarter inch side walls. Air System: Twin 8 inch bore air cylinders with 60 gallon air reservoir, filter, oiler and quick release valves. Fifth Wheel: Oscillating upper plate with standard king pin. Wheels: 8.25 by 24.5 steel disc (8). Tires: 11R by 24.5 Dunlop SP-160 Radials (8). Axles: (2) 25,000 pound Dana 5 inch tubular with 16 and one-half inch by 7 inch cam brakes and oil seals. Lights: ICC and DOT Approved Sealed Beam Lighting System. Suspension: Hutch H-900 Single Point Suspension. Landing Gear: Stiff Leg 4 inch by 4 inch square. Fenders: Full fenders front and rear. Paint: Dupont primer and Dupont White paint. Spare Tire: Spare tire and wheel 11R by 24.5. Tarp: Cramaro Slid-N-Go Tarp. Estimated Weight: 14,750 pounds plus or minus 3 percent. Rear Bumper: Push Block. Dirt Deflector: In front of axle to protect brake chambers. Brake Chambers: Mounted on top of axles. Top Rail: 4 inches by 6 inches by three-sixteenths inch tube. Bottom Rail: 4 inches by 6 inches by three-sixteenths tube. Sides: 54 inches high 12 gauge steel. Hopper: 10 gauge side sheets and inside slope sheets. Mud Flaps: Anti-Sail. The contractor shall provide the equipment as above to the USDA Forest Service, Peaks Ranger District, 5075 N. Highway 89, Flagstaff, AZ 86004 no later than 45 calendar days after award. The FAR provision 52.212-1, Instructions to Offerors-Commerical, applies to this solicitation. 52.212-3 Offeror Representations and Certifications-Commercial Items, and addenda to FAR 52.212-3, the following is added, 52.204-3 Taxpayer Identification Number, 52.204-6 Data Universal Numbering System (DUNS) Number, 52.219-1 Small Business Program Representations, 52.222-70 Compliance with Veterans Employment Reporting Requirements. All offerors shall include with their quotation, a completed copy of provision 52.212-3 and the cited addendas.
