- Potential sources sought to participate in the regional standardization of Laparoscopic Surgical Staplers & Stapling Devices. TRICARE Regions 9 and 10, a military integrated delivery network (IDN), comprised of Army, Navy and Air Force medical treatment centers in California, is considering standardizing and establishing a single source Regional Incentive Agreement for Laparoscopic Surgical Staplers & Stapling Devices. If your company currently holds a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center-Philadelphia (DSCP) for Laparoscopic Surgical Staplers & Stapling Devices your company is eligible to participate in the standardization process. Please respond no later than December 19, 2001 by Email: ruorias@reg9.med.navy.mil and provide the following information: 1. Your company name, address and company?s history 2. Point of contact, telephone, fax and E-mail address 3. Distribution locations 4. What is your DAPA Number? 5. Are your products being distributed by the Prime Vendor, Owens & Minor, serving Regions 9 and 10? If no, why? 6. If you are a distributor, what brand(s) of Laparoscopic Surgical Staplers & Stapling Devices do you distribute? 7. Do you have a customer support through a 1 (800) number? If yes, what is it? 8. Can you provide phone numbers for local sales representatives in Regions 9 and 10? 9. What types of educational tools or materials do you have for this product line? Do you have an In-service Training with C.E.U./C.M.E. units? 10. History of backorders and/or recalls a) Dates and duration b) Cause c) How was information communicated to customers? d) Resolution e) Interim customer support provisions 11. Are your products latex-free? 12. What is your Misfire/Replacement/Damage Policy? 13. What is your upgrade, exchange and restocking policy? 14. Are your products available in full range of sizes? 15. Are your products available in a clear, peel-away, sterile packaging? 16. Are your products disposable, for single-patient use only? 17. Do you have disposable & reusable clips appliers? 18.
